We're hiring for an Integrated Healthcare/CIT Case Manager to join our Co-Response team.

What You'd Be Doing

As the Integrated Healthcare/CIT Case Manager, you would assume a pivotal role in facilitating Health First Colorado (Colorado's Medicaid program) members' access to services and their journey towards wellness as well as providing support to Clinicians integrated within the Pueblo Police Department following up with individuals contacted by CIT clinicians and Police Officers, addressing their case management needs such as resource acquisition, enrollment, home visits, skill development, and other assigned tasks.. Your primary workspace would be in the office, though there may be occasional travel to healthcare facilities and members' residences. Your collaboration with professionals both within and beyond our organization would be essential to ensure that clients receive optimal care.

The case manager would be accountable for the duties below:

  • Appropriate referrals to address mental health, physical health, and social determinants of health needs.

  • Effectively connects members with community resources.

  • Conducts outreach and answers a call line for multiple departments.

  • Conducts home and community visits for members.

  • Ensures members attend scheduled appointments both in the community and at Health Solutions.

  • Identifies stressors and precursors contributing to current situations or crises.

  • Identifies available coping mechanisms and natural supports.

  • Enhances problem-solving, independent living, and socialization skills among members.

  • Supports clients in remaining in their homes and communities, avoiding more restrictive living arrangements.

  • Obtains information and coordinates with community providers when care services are divided.

  • Maintains competency and proficiency with the agency's electronic health record.

  • Assist in goals related to KPI's and APM's.

  • Collaborates with other departments to achieve organizational goals.

  • Performs other job-related duties assigned by the director or designee.

Additional responsibilities specific to the role with the Pueblo Police Department include:

  • Organizing and delivering services according to needs identified by Pueblo Police Department leadership.

  • Providing education on co-occurring diagnoses.

  • Providing consistent support and availability until clients achieve a higher level of functioning.

  • Supporting the reduction of usage of or diversion from more restrictive treatment settings or jail.

  • Communicating with Pueblo Police Department, Dispatch, The Pueblo County Detention Center, and other community partners as identified.

  • Maintaining accurate, complete, and up-to-date records in compliance with company standards.

Physical requirements include the following:

  • Requires standing, sitting, driving, and walking for prolonged periods of time.

  • Occasionally lifts and carries items up to 20 pounds.

  • Requires working under stress in emergency situations.

  • Requires normal range of hearing and eyesight to record data electronically and communicate with clients.

  • Requires occasional driving in adverse weather conditions.

  • Requires frequent use computer operation including keyboard and mouse.

What We'd Like to See in You-The Nice-to-Haves

  • Experience successfully working in multi-disciplinary teams.

  • Experience working with adults with severe and persistent mental illness.

  • Experience providing case management services to adults.

  • Community systems navigation background

  • Spanish language skills

  • CAC training/ CAS or higher

  • Experience working with those experiencing co-occurring disorders.

  • Knowledge of and experience with HUD housing rules and regulations

Qualifications

What We're Looking For-The Must-Haves

  • Baccalaureate degree in social work, clinical psychology, or a closely related field

  • Computer proficiency

  • Experience working with people with substance use, severe and persistent mental illness, intellectual and developmental disabilities and co-occurring disorders,

  • Experience in effective case management/community systems navigation, particularly with the criminal justice and substance use disorder populations

  • Exceptional flexibility, time management, and documentation skills

  • Colorado driver's license, a good driving record, and access to transportation during work hours
