Pay Range: $23.75 - $33.86 USD hourly.

Purpose of Job:

To improve the overall health of the communities we serve by providing continuous, comprehensive and connected health care to Medicare patients enrolled in the Chronic Care Management program through monthly telephonic management of their multiple chronic conditions.

Essential Duties and Responsibilities:

  • Works alongside the Centralized Clinical Services team to identify, outreach and recruit patients into the CCM program.

  • Coordinates care outside of regular office visits for patients enrolled in the CCM program to help address clinical and non-clinical barriers impacting patient health outcomes and to increase patient engagement with Primary Care.

  • Identifies ongoing needs such as health management, social determinants and behavioral health and makes and follows up with appropriate referrals.

  • Creates, implements, and monitors comprehensive and individualized care plans alongside providers and care teams.

  • Documents progress and follows up in Electronic Health Record.

  • Performs other duties assigned, including supporting the CHAS Health Mission and Core Values.

Qualifications:

Education/Experience: One year of health and/or social services experience required.

Completion of Community Health Worker training through the State of Washington required. Medical Assistant Certified credential, Medical Assistant Registered credential, or Licensed Practical Nurse credential issued by Washington State Department of Health required within 120 days of hire. MA-C:

Completion of medical assistant training through an apprenticeship program registered and approved by the WA Department of Labor & Industries or college program accredited by the Accrediting Bureau of Health Education School (ABHES) or the Commission of Accreditation of Allied Health Education Programs (CAAHEP) required.

Certification through the American Association of Medical Assistants (AAMA), National Healthcareer Association (NHA) or National Center for Competency Testing (NCCT) required within 120 days of hire. Washington locations also require a Medical Assistant - Certified credential issued by Washington State Department of Health.

Previous experience in a community health clinic preferred. MA-R: Certification through the American Association of Medical Assistants (AAMA), Certified Clinical Medical Assistants (CCMA) or National Center for Competency Testing (NCCT) required within 120 days of hire. Previous experience in a community health clinic preferred.

LPN: Completion of an Accreditation Commission for Education in Nursing (ACEN), Commission on College Nursing Education (CCNE), or The National League for Nursing Commission for Nursing Education Accreditation (NLN/CNEA) Accredited nursing program required.

Skills: Excellent interpersonal, written and verbal communication skills, with an ability to work effectively with wide range of people.

Must be able to work well independently and with a team. Excellent analytical skills required. Motivational interview skills preferred.

Computer skills required. Valid drivers' license and insurance required. BLS (CPR/AED) required.

Physical Demands:

Required to stand, sit, and be mobile one-third to two-thirds of the time. They are required to use hands to finger, handle or feel over two-thirds of the time, as well as reach with hands and arms.

Climbing or balancing occurs less than one-third of the time; while stooping, kneeling or crouching occurs between one-third and two-thirds of the day. Communicating by talking / hearing occurs over two-thirds of the day. Amount of time spent lifting or exerting force is about 50% for up to 10 pounds and less than one-third of the time up to 25 pounds. Rarely is there a need to lift more than 26 pounds.
