CareCloud is seeking a Chronic Care Case Manager for our North Brunswick office. This position is in-office, full-time.
Essential Duties and Responsibilities:
Provide case management services to patients with chronic or complex conditions
Participate in telephonic interactions with patients and/or care givers to achieve positive outcomes
Assess patient's current health status, treatment plan, services, short/long-term goals, provider options
Develop a care plan using judgment and assessment skills
Follow triage protocols for communicating patient needs
Provide patient education and ongoing monitoring of care plan to evaluate effectiveness, modifying as needed.
Coordinate services required to enhance wellness and manage chronic conditions, improve quality of life, and decrease emergency room visits and avoidable hospital admissions
Advocate for patients, act as liaison, escalate information as appropriate
Complete documentation/reporting in accordance with company procedure
Monitor patient vital signs and taking action where needed
Document all client communications in an accurate, efficient, and timely manner
Display excellent customer service skills with patients and partners
Adhere to documentation protocols and best practices for complianceand HIPAA
standards
Required Knowledge, Skills and Abilities:
Experience working with Electronic Health Records (EHR) software
Experience working with chronically ill patients preferred
Ability to recognize condition specific symptoms
Ability to maintain confidentiality and follow HIPAA guidelines
Ability to handle difficult and sensitive conversations; active listener
Comfortable assessing status of patient remotely; Previous phone experience
Excellent written and verbal communication skills; Spanish language proficiency a plus
Ability to work in a fast pace environment and multi-task
Proficient in MS Windows, Word, Excel and Outlook
Team player
Professional and friendly demeanor; ability to develop and build trusting relationships
with patients and partners
Accomplish tasks with minimal supervision.
Organized; thorough documentation skills
Solid time management skills
Education and Experience:
1+ years of experience in the healthcare industry; preferably in a provider's office, facility or medical billing/RCM company
Certified/Registered Medical Assistant, Medical Coder or other qualified background
