RESPONSIBILITIES AND COMPETENCIES

Rights, Confidentiality, and Ethics Functions

  • Complete training in and comply with all Rights, Confidentiality, and Ethics policies.

  • Demonstrate respect for and awareness of cultural differences.

  • Compliance with ethical and human treatment of staff and individuals served, as evidenced by assessment during supervision.

Safety Functions

  • Support all efforts to ensure the safety and well-being of individuals, staff, and the program. Provide interventions to assist the individuals served in feeling safe. Observe all risk precautions and safety plans for each person.

  • Comply with all safety regulations and policies, including transportation, site, and community safety policies.

  • Complete Risk and Home Safety Assessment prior to entering locations, and upon new information received regarding home of individuals served.

  • Competent in safety activities and take action as indicated.

  • All required safety tasks are completed in a timely manner.

Care, Treatment, and Services Functions

  • Complete, in a timely manner, all required assessments, treatment plans, and additional clinical documentation for each individual served thoroughly, with use of trauma informed practice. Ensure that assessments are thorough and inclusive of family and medical history.

  • Based on results of assessment, develop a person-centered treatment plan (IAP) in collaboration with the individual served, parent/guardian/caregiver(s) if applicable, and with required consent, in consultation with other providers.

  • Provide individualized treatment that may include working with the individual served, entire family, or a subset of the family, to implement focused, evidence-based interventions to advance therapeutic goals or improve ineffective patterns of interaction.

  • Facilitate treatment planning discussions to include input of all members of Treatment Team, family members, agency workers, and the individual. Monitor progress of individual served in meeting goals and objectives. Additionally support that treatment through facilitation with other team members of access to community resources.

  • At individual's and identified need accompany the individual to meetings about the individual's behavioral health treatment needs in schools, day care, foster homes, and/or other community-based locations.

  • Participate in Discharge process, including providing support for the individual served and family, work with agencies to identify most appropriate aftercare services (if applicable); work with aftercare provider to facilitate transition.

  • Support individual served with accessing other community resources that are identified as needs.

  • Facilitate groups that support individuals and families that are survivors of community violence and/or homicide.

  • Provide PFA and respond to on-call emergencies for crisis response when requested.

  • Provide coverage 24/7 for Trauma hotline when scheduled for on-call support. CTS Competencies: (as assessed through Senior Program Manager's review).

  • Demonstrate familiarity with each individual served' s history, primary issues, measurable goals and objectives as evidenced by timely and thoroughly filled out assessment and treatment planning documentation.

  • Diagnoses are based on assessment and DSM-V criteria and ICD 10.

  • Interventions are clear and appropriate for the individual served and family members.

  • Therapies are structured and well planned, address treatment needs, and include appropriate interventions to assist youths in achieving their goals and objectives.

Documentation and Information Management Functions

  • Complete all documentation in a timely manner according to policies and procedures, including but not limited to CATS, CANS, Satisfaction surveys, Comprehensive Assessments, IAPs, etc.

  • Attend program staff meetings. Meet regularly with Program Director, and/ or supervisor to discuss clinical issues. In conjunction with Program Director, prioritize activities and ensure that all are completed.

  • Attend meetings as required by referring agencies or community partners.

  • Maintain a caseload with number of clients required by program director and program/ site requirements.

  • Provide support to individuals identified in the community in a manner consistent with the program's requirements.

  • Maintain productivity expectation as indicated and required by program director, indicated by properly entering progress notes within 72 hours of services rendered.

Documentation and Information Management Competencies

  • All documentation is completed by specified timelines, as evidenced by record audits.

  • All documentation is completed thoroughly and appropriately, as evidenced by record audits.

Leadership and Relationships Functions

  • Assume leadership role in Team meetings for assigned individuals, while collaborating and hosting meetings, and forums for all team members.

  • Create strong liaison relationship with individuals and agencies; return calls in a timely manner.

  • Take part in an on-call rotation, as required by program and site-specific rules.

  • Seek consultation and direction through established lines of supervision resolving situations that interfere with treatment and work relationships.

  • Relationships with all individuals served and community partners are positive and collaborative in nature.

Staff Development Functions

  • Attend all mandatory or assigned in service training provided to comply with annual training requirements.

  • Develop Professional Development goals with Senior Program Manager of Leadership Development & Protocol Management. Seek opportunities that will promote professional growth.

  • Complete CEU training to assure maintenance of license (if applicable).

  • Complete all required trainings yearly, including maintenance of required certifications, as assessed through audit of training records.

  • Complete other duties as assigned by the Senior Program Manager.
