Overview
The Child and Family Team (CFT) Facilitator's primary responsibility is to facilitate Child and Family Planning Meetings using wraparound principles that are family-centered, individualized, strengths-based, and outcome-focused. The program's goal is to facilitate Child and Family Team Meetings with a family-centered approach in order to assess, plan, intervene, monitor, and refine services over the course of the Departments involvement with the family. The CFT Facilitator role is to engage family members in the decision-making process and lead the team in discussion to work together to identify each family member's strengths and needs to develop a child, youth, and family-centered decision.
