Provide proactive intervention and care coordination, to include treatment planning, assessment, monitoring, educating and referrals to individuals in identified population. Provide leadership and support of organization efforts focused on improving the five functions of quality: outcomes, patient experience, clinical resource utilization, regulatory and failure analysis.
Collaborate with care team members to provide leadership to all staff in order to provide a direction and plan of care for patients
Coordinator/ Navigator for CHF/AMI for Cardiology Registry and Accreditation Programs: Work in collaboration with internal and external stakeholders on cardiology certifications and related activities and to assure compliance with all standards for each certification.
Serve as a clinical resource and expert in the area of specialization to provide consultation to healthcare providers and healthcare consumers
In collaboration with the Clinical & Medical Directors provide continuous project management leadership and oversight in the planning, organizing, implementation of quality goals for all assigned Key Services and projects.
Serve as a process improvement expert that requires clinical expertise as well as expertise in facilitation, change management and quality improvement methodologies.
Work with assigned Key Services, various departments and team members in the analysis of data related to outcomes, patient experience, clinical resource utilization, regulatory, failure analysis and provide expertise in identifying trends patterns and suggests developing process improvement plans.
Provide leadership and oversight to the data collection of various pay for performance programs and data registries to ensure the data is retrieved, analyzed and disseminated in usable formats for various levels of understanding.
Perform other duties as assigned.

Excellent interpersonal and communication (oral and written) skills are needed in order to interact effectively with physician, nursing staff, and other hospital departments. Ability to demonstrate customer focused interpersonal skills in order to interact in an effective manner with customers. Basic computer, data entry, typing, filing and organization skills required. Technical skills to develop computer tracker models, access and manipulate computer data and understand utilize statistics. Must be proficient in Microsoft office including word, excel, power point, and vision.
Abilities: Ability to work extended hours beyond normal work schedule to include, but not limited to, evenings, weekends, extended shifts and/or extra shifts, various locations, sometimes on short notice.
Environmental Conditions: Potential exposure to fumes, dust, gases, and odors. Frequent changes in temperature may occur. Possibility of exposure to communicable diseases.
