NorthPoint Health and Wellness Center is seeking a Senior Chemical Health Counselor to be a part of the Medically Assisted Therapy clinical team providing alcohol and other drug education, peer support, counseling, and care coordination

Location and hours:

This position is hybrid and will be performed both on-site a minimum of 4 days a week at NorthPoint Health and Wellness Center, 2220 Plymouth Avenue North, Minneapolis, Minnesota with the opportunity for remote work 1 day a week once employee has completed training period. This position will be Monday through Friday, with typical hours of 8:30 a.m. to 5 p.m. While this position is designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin.

New employees who are hired into remote or hybrid positions between January 2, 2022, to December 31, 2024, will receive $500 toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment.

In these positions, you will:

  • Conduct chemical use assessments with clients and recommend appropriate treatment services.

  • Conduct intake, orientations, supportive counseling and discharge planning activities

  • Conduct educational classes and group meetings, including family members and community.

  • Monitor client progress and maintain a variety of records and prepare progress reports.

  • Coordinate services with client's other treatment providers.

  • Provide coordination of care with substance use disorder (SUD) providers & treatment facilities, provide health education and recovery support to Hennepin Health members regarding SUD, and complete and maintain documentation in the electronic health record (Epic) and documentation required by State (DHS) and treatment facility guidelines.

  • Collect and maintain program data and reports.

  • Work with interdisciplinary care teams in the hospital, community, and at SUD treatment centers to meet the needs of HH members, with minimal on-site supervision.

Need to have:

  • A bachelor's degree or higher from an accredited college or university that includes 18 or more semester (or 27 or more quarter) credits in alcohol and drug counselor education, and completion of an 880-hour alcohol and drug counselor practicum, or equivalent training that meets the requirements for Licensed Alcohol and Drug Counselor (LADC) licensure.

  • Three or more years of professional level work experience in a chemical or behavioral health setting.

  • Licensure as an Alcohol and Drug Counselor by the Minnesota Board of Behavioral Health and Therapy (BBHT).

Nice to have:

  • Experience:

  • Collaborating with physicians, nurse practitioners, nursing staff and primary care providers to determine appropriate care.

  • Working with adults with mental health and/or substance use issues.

  • Providing resources and working with those individuals facing homelessness.

  • Using electronic health records, such as Epic.

Knowledge of co-occurring and mental health diagnoses, symptoms and treatment including harm reduction model.In-depth knowledge and understanding of recovery, recovery maintenance, and relapse related needs.The ability to provide excellent customer service to crisis situations.Skill in motivational interviewing; awareness of Person-Centered Thinking (PCT) concept.
