GENERAL SUMMARY AND SCOPE

Under the supervision of the director or designee, maintains medical record integrity by preparing documents for scanning into the electronic document management system. Examines each page of the patient paper chart and verifies patient identification on each page. Ensures that barcodes and form identification are on each form and all forms are ready to be scanned. Retrieves confidential patient records; assists physicians and authorized medical center staff in locating records, and answers telephones and greets visitors.

ESSENTIAL TASKS, DUTIES AND RESPONSIBILITIES

  • Prepares charts for scanning including verifying mnemonics on each page and ensuring proper document and date order, and account number. Follow through on missing pages by checking the loose sheets and with the nursing floors.

  • Resolves issues identified in quality control process. Identifies and resolves issues with non-standard or problem forms. Identifies any forms not in Forms Management Dictionary and routes to Lead Health Information Analyst/Director for follow-up.

  • Reviews scanned charts to ensure there is no outstanding dictation and that all documentation is signed by the physician. Resolves issues related to quality and accuracy. Re-scans as needed. Files complete charts in the correct location.

  • Follows established customer service guidelines when interacting with customers on the telephone or in person. Demonstrates ability to work as a team within the Health Information Management Department.

  • Performs clerical duties related to the Health Information Management Department including answering phones, taking messages, file chart components and communicating information to the appropriate person accurately and professionally.

OTHER TASKS, DUTIES AND RESPONSIBILITIES

  • Retrieves charts from nursing floors and Digestive Health Department.

  • Maintains the chart locator application.

  • Performs scanning duties as needed and helping with loose sheet scanning.

  • Participates in department performance improvement activities as assigned and attends required training.

KNOWLEDGE, SKILLS AND ABILITIES

  • High school diploma, or equivalent.

  • Knowledge of HIPAA rules and regulations.

  • Completion of a medical terminology course is helpful.

  • Working knowledge of office equipment including personal computer/keyboarding, copy machine, fax machine and multi-line telephones.

  • Knowledge of medical record format and content for all visit types. Ability to examine the record and verify patient identification. Ability to examine a form and determine its proper placement within the record. Ability to identify non-standard forms and determine action required.

  • Knowledge of medical records functions.

  • Willingness to work as a team player within the Health Information Management Department.

  • Ability to effectively communicate with physicians and other staff members.

  • Ability to maintain and project a positive impression of the Medical Center at all times.

  • Ability to handle telephone and face-to-face inquiries and problems using excellent customer service skills.

  • Ability to understand instructions in both written and oral form.

  • Punctual attendance at assigned work location is required.

  • Ability to work in a safe and efficient manner and maintain an accident free work place, including ability to demonstrate a working knowledge of Medical Center emergency codes.

  • Ability to comply with OSHA regulations and CDC standard and transmission based precaution recommendations and to utilize proper personal protective equipment. Ability to comply with provisions of applicable S.D.S. forms. Successful completion of a 90-day get-acquainted period.

  • Successful completion of a 90-day probationary period.

ESSENTIAL TASKS, DUTIES AND RESPONSIBILITIES Performs Cardiac Ultrasound Testing (echocardiography) - Interacts with and educates patients regarding echocardiography - Performs scans; using the M-mode, 2-D (color flow), and Doppler according to established protocol - Informs physicians of observations, as needed - Completes documentation utilizing echocardiography worksheet - Collaborates with RN to perform contrast ECHO study when deemed necessary - Ensures documentation of study - Utilizes left lateral decubitus position for exam - Identifies suboptimal images and utilizes strategies to maximize image quality - Maintains performance level of suboptimal exams below established benchmark - Ensures timely completion and distribution of dictated and transcribed reports - Supports accreditation standards and assists in maintaining ICAEL accreditation status Preparation for Cardiac Stress Testing - Interacts with and educates patients regarding cardiac stress test - Reinforces and ensures patient understanding of procedure - Attaches/disconnects electrodes; prepares set-up of stress testing equipment - Obtains blood pressure and assists patients during testing. - Records data on assessment sheet Performs Holter Monitor Service - Interacts with and educates patients - Attaches/disconnects electrodes/monitor - Programs Holter monitor and ensures functioning Cardiac Event Monitor - Applies and disconnects cardiac event monitors. - Provides patient education related to maintenance of monitor and transmission of cardiac events. - Manages data in software program and completes report preparation. Page 2 of 5 Associated Duties - Assists with quality improvement efforts - Develops and conducts education programs in areas of expertise for staff and patients. - Performs 12-lead EKG and completes required paperwork and documentation Applies ambulatory blood pressure monitor and provides patient with education related to monitor. Performs other duties, as assigned. KNOWLEDGE, SKILLS AND ABILITIES Associate Degree in Cardiovascular Technology, or a minimum of 3 years experience in cardiopulmonary specialty procedures. Cardiac Echo certification is required. BLS required. Strong background in cardiopulmonary anatomy, physiology and pathophysiology. Ability to identify cardiac arrhythmias and dysrhythmias. Ability to perform the following cardiopulmonary specialty procedures: a. Cardiac stress testing preparation b. 12 lead EKG's c. Holter monitor application d. Cardiac event monitor application Proven good work attendance record. Punctual attendance at assigned work location is required.
