Overview

Our Charge Registered Nurse (RN) performs the functions of assessing, planning, implementing and evaluating the care for the assigned patients on the unit during a shift. As a charge nurse, the RN is responsible for supporting all assigned personnel, supplies and equipment and for the promotion of collaboration with all members of the health care team.

The primary goal for the Charge RN is to ensure positive patient care outcomes are achieved. Our RN works with other members of the health care team, family and/or caretakers to provide compassionate care to the patient. Our RN provides guidance, assists with orientation and oversight for the RN's employed by MLCRP.

Qualifications

Education:

  • Graduate of an accredited school of nursing. BSN qualification and nursing specialty certification preferred.

Experience:

  • Experience in acute and/or forensic settings preferred.

Licenses/Certifications:

  • Must have a current, unrestricted license to practice as a Registered Nurse (RN) in the state of practice, or be eligible for same through reciprocity with other states.

Responsibilities Ensures the performance and documentation of a comprehensive nursing and physical assessment of the patient with data gathered through interview, observation, inspection, auscultation, palpation and reports. Determines patient problems, age-based needs, desired outcomes and nursing interventions based on the assessment.

The written plan of care will include clinical, teaching and discharge components. Implements the interventions defined in the plan of care so that desired outcomes can be achieved. The RN will ensure appropriate patient and health education.

The RN will individualize interventions with consideration of patient's age; ensure the documentation of all interventions. The RN will delegate appropriate interventions to other team members. Provides oversight for RN's employed at MLCRP.

Reviews documentation of nursing care, reviews administration of medication and evaluates treatments provided to patients as ordered. Evaluates nursing outcomes and makes revisions to the nursing plan of care as required. The RN will ensure the reassessment of the patient in accordance with departmental policy.

Leads and participates in performance improvement initiatives for the Facility, specifically related to nursing functions. Aggregates and analyzes information related to nursing practice to identify trends. Participates and performs in nursing peer review to evaluate quality of nursing care.

Practices principle of risk management, safety and infection control. Intervenes in and reports directly in situations which are immediately harmful to the patient. Ensures the completion of all required documentation related to incidents or situations in which an unusual event has occurred.

Utilizes appropriate interpersonal skills when collaborating with organizational health care team members. Participates in utilization review, discharge planning and case management to ensure appropriate level of care and utilization of resources. Accurately transcribes medical orders and ensures appropriate procedures are followed for transcription and verification of medical orders.

Establishes intra- and interdisciplinary collaborative relationships with all members of the health care team. Serves as primary liaison with physicians related to the care of the patient. Participates in orientation and continuing education of peers and other members of the health care team.

Responds to changes in the work setting and in the schedule in a collaborative and flexible manner. Demonstrates accountability for professional practice as evidenced by: Initiation of plans to solve department level problems and to improve patient care.

Professional practice within legal and regulatory parameters. Participation in patient and healthcare committees. Assesses needs of patients and delegates appropriately to team members.

Organizes and prioritizes elements of patient care for self and team. Models professional behavior reflective of the nursing care philosophy of safe and compassionate care. Ensures availability of adequate supplies and material resources to provide care for patient.

Maintains annual clinical and administrative competencies for nursing care. Completed HIPAA training and maintains confidentiality of patient and employee information and records. Advocates for patient's rights.

Maintains the image of the WA Cascade Cottage and work area as a safe, clean, efficient, caring organization by demonstrating a team approach which produces an environment conducive to patient, family and team well-being. Demonstrates loyalty and commitment to the organization. Understands that safe and compassionate care is the number one priority.

Must be able to apply principles of critical thinking to a variety of practical and emergent situations and accurately follow standardized procedures that may call for deviations. Must be able to apply sound judgment beyond a specific set of instructions and apply knowledge to different factual situations. Must be alert at all times; pay close attention to details.

Must be able to work under stress on a regular or continuous basis. Perform other duties as assigned.

Under the supervision of the Charge Nurse(s), directs and guides patient teaching and activities that commensurate with his/her education and demonstrated competencies. Qualifications Education for Registered Nurse: * Graduation from an accredited School of Nursing Experience for Registered Nurse: * One (1) or more years clinic experience preferred Licenses/Certifications for Registered Nurse: * Current licensure as a Registered Nurse (RN) in the state in which you work, or a valid compact state nurse license * Current CPR Certification Responsibilities Reviews nursing plans to ensure data collected, nursing diagnoses, and proposed nursing care plans are relative and confirm with authorized policy and procedures. Reviews medical records to ensure nursing practice provided is within boundaries of State guidelines and all nursing recordings are in legible SOAP format to include the date, time, signature and discipline. Ensures all referrals for consultation, treatment or resolution of problems are valid and applicable to the documented nursing plans. Monitors the nursing service, ensuring the provision of progressive patient care plans, employing the total team approach including patient teaching and self-care. Assists the DON to ensure that the technical and clinical skills of the Registered Nurses are current that they function within the scope of their licensure, and nursing actions are not delegated to another provider for which that person is not prepared or qualified. Follows through and documents all findings, corrections or counseling for all nursing incidents, medication errors, patient complaints and forwards to the Director of Nursing for disposition. Identifies potential problems, establishes priorities, initiates effective problem solving based on clinical knowledge and informs Director of Nursing of proposed corrective actions. Ensures Director of Nursing is informed of any concerns raised by the nursing staff regarding the questionable lack of appropriate medical intervention for the realization of established patient goals. Maintains composure in stressful situations, aware of own limitations and seeks guidance as necessary from the Director of Nursing. Assumes administrative and clinical call on a scheduled basis when requested. In the absence of the Director of Nursing, will assume the responsibility of his/her position when requested. Develops monthly staffing schedule for review and approval by the Director of Nursing. Ensures the staffing levels adhere to the contractual requirements and staff requests for vacation or special time off is granted whenever possible. Assists the DON as needed to ensure an accurate account of monthly staffing and statistics as required by contract. Assists in the orientation and on-the-job training for all new employees assigned to the unit. Ensures employees are assigned duties and responsibilities to the level of their expertise, licensure or certification. As delegated by the Director of Nursing, annually evaluates performance of supervised staff according to established policy and procedure. As assigned conducts staff meetings for all supervised health care providers to identify problems, propose solutions, set goals. Assists in the collection of monthly statistics as assigned. Reviews, monitors, and maintains perpetual inventory of all contraband and tool counts; investigates and reconciles any discrepancies and reports findings to the Director of Nursing. Cleanliness, maintenance, sanitation and appearance of unit and appropriately notifies security of any infractions needing correction or attention. Coordinates patient care with other departments, to include but not limited to, intake admission, transfers, infirmary admissions, off-site hospital admission, in-house referrals, off-site referrals, periodic and annual physicals, mandatory release, parole, and patient teaching. Assists the DON in ensuring that appropriate levels of stock for pharmacy, medical supplies and laboratory supplies are maintained. Provides for preventive maintenance for all medical equipment as appropriate, and initiates procedure for repair as indicated. Assists in the maintenance of a professional atmosphere on the nursing unit as well as a high standard of patient care. Sets good example for the staff through regular attendance, arriving promptly on time for duty, being appropriately dressed well groomed, adhering to the dress code, and displaying the identification badge appropriately and being neat and well groomed. Attends monthly institutional staff meetings as directed by the DON or Administrator. Other duties as assigned by the Health Service Administrator or Director of Nursing.
