Sombrillo Nursing and Rehabilitation, an integral part of Los Alamos Retirement Community, is currently seeking a qualified and professional Charge Nurse (RN) to provide exceptional care for our skilled nursing facility patients. With a newly updated assisted living and skilled nursing facility, we offer a broad range of services to our residents. Please consider becoming a valued member of our team!

Purpose: The Charge Nurse is responsible for professional nursing services and care provided to assigned residents and provides overall supervision of resident care activities.

Responsibilities & Duties:

  • Complete all required record keeping forms/charting upon residents admission, discharge, transfer
  • Receive verbal orders from attending/alternate physician and transcribe
  • Initiate Resident Care Plan upon admission
  • Follow Interdisciplinary Team Plan of Care for each resident
  • Supervise assigned care staff
  • Complete documentation on each individual resident as required to reflect the care provided as well as the residents response to the care
  • Communicate with residents, designated support member and physician on change of condition and medical care
  • Administer medications and professional services such as catheterization, tube feeding, suction, applying and changing dressings
  • Communicate closely with Interdisciplinary team members


Requirements &


  • Current RN license from the New Mexico Board of Nursing
  • Detail-oriented
  • Experience in skilled nursing/or sub-acute care setting preferred, but not required
  • Excellent customer service skills; dedication and commitment to providing quality healthcare services
  • Ability to operate effectively and efficiently in an enthusiastic, resident- focused, work environment

Provide clinical leadership for other staff and serve as a clinical resource for staff. Serve as a role model to promote a positive work environment and quality patient care while utilizing SRMC Values and promoting SRMCs mission and vision. Enforce and monitor compliance with SRMC and Emergency Department policies, procedures & guidelines. Perform direct patient care and/or accept a patient assignment as needed. The job involves a moderate degree of complexity. Patient care assignment may include neonate, infant, pediatric, adolescent, adult and geriatric age groups. Essential Functions: * POLICIES AND PROCEDURES - Maintain established departmental policies and procedures, objectives, and quality assurance programs * PROFESSIONAL DEVELOPMENT - Enhance professional growth and development through participation in educational programs, reading current literature, attending in-services, meetings and workshops. * Report potential and actual patient safety, concerns, medical errors and or near misses in a timely manner thru the chain of command and document within the PSN system. * Establish a compassionate environment by providing emotional psychological and spiritual support to patient, friends and families. * Promote patients independence by establishing patient care goals; teaching patient, friends, identified lay caregiver and family to understand condition, medications, and self-care skills; answering questions. * Maintain a cooperative relationship among health care teams and resolve patient problems and needs by utilizing multidisciplinary team strategies. * Active participation in nursing strategic initiatives such as quality improvement and monitoring, TJC compliance, recruitment and retention of staff and patient and employee satisfaction * Delegate tasks and supervise the activities of other licensed and unlicensed care providers. * Monitor and initiate corrective action to maintain the environment of care including equipment and material resources. * Ensure ability to assess, initiate and document nursing assessment, plan of care, interventions, evaluation and re-evaluation of patient status * Ensure ability to document and communicate pertinent information using computer and/or paper documentation tools in a timely and accurate manner. * Utilize independent judgment assign nursing personnel and staff to specific patients. * Designate shift and schedule assignments, call in staff, send staff home and monitor attendance; assign duties in accordance with the plan of care, patient needs, and within the scope of staff qualifications and licensure/certification/registry and monitor completion of duties according to departmental and hospital policies. * Receive and accept compliments, complaints and feedback from patients, visitors, staff, providers and outside entities, then initiate appropriate follow-up actions and communication based upon the particular circumstances * Act as a clinical resource and role model for department personnel. * Provide and assure emergency department clinical treatment in a manner that meets or exceeds established standards of care * Perform miscellaneous job-related duties as assigned.
