Opportunity

We are looking for a Charge Nurse to render or assign resident care according to the needs of the residents under the direction of the supervisor and or director of nursing. Maintain a home like, person centered care environment for all residents. Effectively supervise all staff responsible for resident care. As an interdisciplinary team member and supervising others to meet the physical, psychological, and social needs of the resident.

Qualifications

Education:

  • Graduate of an approved school of nursing

Licenses/Certifications:

  • Current nursing license

  • Maintains current state registration and ongoing education

Responsibilities

  • Responsible for all nursing service and its administration and clinical supervision

  • Responsible for the implementation of nursing objectives, standards of nursing care, nursing policy and procedure manual, and reviews and revises such as necessary and at least annually.

  • Interprets all nursing policies and procedures to nursing personnel. Determines that nursing care standards and nursing policies and procedures are being followed according to written guidelines

  • Supervises and gives clinical guidance to nurses and other nursing staff (PCA, C.N.A., CMT etc.) as necessary.

  • Makes rounds to assess resident and staffing needs and visits residents each work day.

  • Works under the direction of director of nursing to assure that all nursing personnel complete the required orientation and attend the necessary in-service training, and that appropriate documentation is maintained.

  • Responsible for nursing assessment on monthly or as needed basis, documenting assessments, notifying physicians and families of change in condition, obtaining appropriate orders and implementing order. Responsible for supervising personal hygiene of residents.

  • Responsible for seeing that all the necessary written documents such as Admission Nursing Assessments, Nursing Notes, Over-All-Care Plans and other required information is kept current in the resident record system. (Records are kept current following facility's policies, ASI standards and/or State and Federal regulations.

  • Responsible for assisting director of nursing in keeping facility standards up to state and federal and ASI requirements and assisting with any and all inspections. Team member in planning, evaluating and providing safe home like environment and quality of life for residents.

  • Responsible for determining that the medication systems meets the needs of the resident, is an effective type administration system and is set up according to State regulations and pharmaceutical guidelines.

  • Keeps administrator informed of significant changes in residents conditions, transfer of residents to the hospital, expirations or any pertinent information regarding the residents and or employees.

  • Assists in determining the type of residents to be admitted to the facility using established nursing care criteria as a guideline. Makes decisions as to placement of residents and transfer to other rooms as necessary.

  • Communicates with Administrator schedules and conducts nursing staff meetings and maintenance records of such.

  • Assesses supply and equipment needs for nursing department and sees that adequate stocks are maintained.

  • Determines that incident reports are made out as necessary or as indicated and reviews such on a routine basis.

  • Available for call when needed by the staff members on duty in matters pertaining to nursing care.

  • Performs any other duties as may be necessary as specified by the director of the facility.

Position Summary: The wound care nurse is specialized in managing and treating the disorders that affect the genitourinary, integumentary or gastrointestinal systems and treat the wounds by giving care and necessary treatment to the patient under various settings. However the wound care nurses are considered to be a part of acute care teams, they are a vital component to this team. The wound care nurses are trained to treat the wounds commonly found in long-term or acute health care. Essential Functions: Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. * The wound care nurses have to treat the skin reactions of the patients * They have to treat or manage vascular ulcers, diabetic ulcers and all the other type of ulcers * Treat all the types of open wounds, scars or skin infections * Preventing the skin breakdown * Taking proper measures to prevent the wound from getting worse and providing proper care to the patient * Treating and assessing the draining wounds and fistulae problems * Practicing the comprehensive wound management to control odor and performing causative factors * The wound care nurse have to assess all the nutritional needs to the patient and providing the topical therapies to them * They have to design pouch systems for draining fistulae and wounds of the patient * Educating and supporting the patient morally and emotionally is also a part of the responsibilities of a wound care nurse * They also have to provide emotional support to the patients family and educate them of all the necessary care and wound * They have to learn to provide all the instructions to dress the wound and treat them in the right manner * Implementing preventive measures in order to reduce the incidences of ulcers and wounds is also an important part of the duty of a wound care nurse * Educating the patient about the wound or ulcers and make him aware of the treatments done on him * Examining the wounds or problem of the patient and preparing the list of tests or examinations needed * Providing proper setting to the patient to treat him * Helping and assisting the doctor and giving him all the help needed * To coordinate with the medical team to provide proper treatment to the patient * Examining the recovery of the patient and altering the medications as needed. * Apart from the above-given responsibilities and duties of the wound care nurse, they also have to practice all the basic nursing duties when needed. They are the specialists who have to treat, monitor and assess the patient's wounds to provide them better health management. Work Environment Indoor with adequate lighting and ventilation, carpet, tile, concrete floors. May include outdoor or public activities or attending recreational events with residents. Physical Demands The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms. OSHA Exposure: Category 1, possibility of exposure on daily basis. Required use of personal protective equipment Safety Equipment:When administering resident care, gait belt, lumbar support, two man lift policy, and use of mechanical lifts in noted facilities Position Type/Expected Hours of Work This is a full-time position. Hours can vary depending on coverage and demands of the community. Qualifications/ Education/Experience/Skills: Education: * Must be an LPN and have experience in Wound Care and skilled nursing. * Comply with both federal and state guidelines, including all licensing requirements. Experience: * One (1) or more years clinical experience SNF preferred * Prefer 2 or more years in long term care facility or assisted living.
