Job Description:




At the Highest level, Catholic Hospice Employees must (a) be committed to Hospice philosophy and mission and (b) provide outstanding customer service, aligned with the Customer Service Guidelines, to both internal and external customers. Additionally, be able to promote an awareness and understanding of positive ethical and moral principles consistent with the mission, vision, and values of Catholic Hospice and those required by law. The Spiritual Counselor acts as a community liaison for patients linking them to local parish/congregation/ synagogue, while providing counseling in cooperation with the Interdisciplinary Team's plan of care.

  • Ability to interact with the public and other employees in a positive manner and promote a cooperative working environment.

  • Ability to handle sensitive information, maintain confidentiality, and coordinate several projects at the same time.

  • Ability to coordinate several projects at the same time with competing demands and tight time lines.

  • Demonstrate adherence to Catholic Hospice, Inc. polices and procedures and pertinent State and Federal Rules and Regulations.

  • Computer literacy required with proficiency in word processing and general computer applications.

  • Provide accurate and appropriate documentation and submit daily.

  • Ability to assess spiritual factors related to patient care and intervenes appropriately.

  • Provides explanation of spiritual aspects of care to interdisciplinary team members.

  • Acts as liaison with professional religious community by making reasonable efforts to arrange for visits of clergy and members of religious organizations in the community to patients who request such visits.

  • High comfort level in dealing with terminal illness.

  • Demonstrates knowledge of hospice levels of care, Medicare and Medicaid Hospice Benefits by providing appropriate documentation when the need arises.

  • Exhibit respect for individual religious preference/practice.

  • Exhibit high comfort level in dealing with terminal illness.

  • Consistently able to work calmly and maintain effectiveness in situations of high stress.

  • Maintaining the ability to extend hospitality and patience while assisting patients, families, physicians and peers under stressful circumstances.

  • Educates religious community to the collaboration of Hospice and clergy.

  • Ability to travel to any location throughout South Florida as needed and expand territory as business needs dictate.

  • Embodies principles of CHS Corporate Compliance Program and Code of Conduct at all times.

  • Complies with Code of Conduct in all relationships with clients, staff members, and visitors.

  • Adheres to the Code of Conduct in all daily activities and decisions ensuring that ethical standards and applicable laws and regulation are met.

  • Responsible for the integrity and accuracy of organizational documents and reports.

  • Reports violations or suspected violations of the Code of Conduct appropriately.

  • Maintains the policy of confidentiality.


  • Makes visits as necessary according to assessed needs, within established time frames.

  • Provide spiritual support of patient/family on individual or group basis. Support services must be organized to meet the needs of patients, families, and others that may have vital interest in the patient's well-being. The services must conform to the plan of care. Patient/family records should indicate the extent to which support services are used. The plan of care should be consistent with the patient/family needs.

  • Administers sacraments (or arranges for them to be administered) as requested and performs funeral or memorial services upon request by the family.

  • Participates as an active member of the interdisciplinary team identifying and addressing spiritual matters through:

  • case consultation and case finding

  • collaboration in interdisciplinary team meeting

  • patient/family visits

  • establishment of a spiritual care plan

  • Coordinates spiritual services within structure of team.

  • Insure documentation includes patient's religious preference and assistance offered.

  • Provides spiritual counseling to meet needs in accordance with the patient's and family's acceptance of this service, and in a manner consistent with patient and family beliefs and desires

  • Provides Team Manager with weekly schedule of patient appointments and follow-up documentation.

  • Develops and assists with educational services in the community.

  • Develops, supervises and evaluates the annual Hospice Memorial Services.

  • Ensure effective communication with Interdisciplinary Teams.

  • Provide orientation on a regular basis to new employees.

  • Participate with on-call rotations.

  • Performs other duties as assigned by supervisor.


Ability to read, analyze, and interpret common information, reports and medical documents. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the Healthcare industry. Ability to effectively present information to management, public groups, referral sources or patients and caregivers. Ability to speak and write Spanish is strongly preferred.

Field positions will be exposed to various unfamiliar conditions which may include, but are not limited to: patient homes, nursing homes, assisted living facilities, doctor's offices and hospitals. Job Requirements


To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.   • Membership in either A.C.P.E. or N.A.C.C. desired.

  • Four (4) units of CPE (Clinical Pastoral Education) or equivalency are preferred

  • Recognition or confirmation by your church, congregation, or synagogue as such and in good standing.

  • Master or Doctorate Degree in Divinity

  • At least one year's experience in related area.

  • Preferably certified by an appropriate agency.

  • Excellent communication skills, preferably bilingual (English / Spanish).

  • Basic understanding and knowledge of medical concerns and how they relate to the person's overall physical, mental, and spiritual health.

  • Must be 18 years or older

  • Pass the drug test

  • Ability and willingness to work a flexible Full Time schedule

  • Must be able to use facsimile, write and transcribe information accurately.

  • Must be able to provide own transportation to various locations in service areas as required by duties
