Job Description:

Description: The Certified Pharmacy Technician - Inpatient assist the pharmacist(s) in storing, preparing, distributing, reconciling, and charging of all medications to the appropriate nursing areas serviced by the pharmacy. The technician works under the direct supervision of the pharmacist and will only perform activities that are in compliance with the Texas State Board of Pharmacy Rules and Regulations.

Typical Duties:

  • Accurately and efficiently prepares and labels medications. Always obtains a final check from the pharmacist before dispensing/delivering a medication or releasing any compounded parenteral medication.

  • Accurately prepares I.V.'s, utilizing aseptic technique, under the supervision of a pharmacist, in accordance with USP <797> Guidelines and Texas State Board of Pharmacy Rules and Regulations.

  • Makes timely rounds and accurately and safely transports medications and other pharmacy supplies to assigned areas. Maintains confidentiality of patient information.

  • Enters patient and departmental charging information into the computer system.

  • Assists in maintaining unit dose readiness of bulk products. Verifies all barcodes scan accurately.

  • Replenishes stock in the automated dispensing system, crash cart trays, and specialty medication boxes accurately and timely.

  • Monitors and ensures adequate inventory is maintained in all areas. Notifies the inventory control technicians of products that need to be reordered. Manages drug supplies in a cost-effective manner and identifies opportunities to minimize waste.

  • Performs and documents quality control activities, including monthly medication storage area inspections, and completes information on all required log sheets.

  • Performs daily ADS routines (outdates, discrepancy resolution, loading/unloading, etc.) and troubleshoots all ADS issues (cubie/drawer failures, temperatures out of range, interface issues, etc.)

  • Ensures medication management processes meets all legal and regulatory requirements.

  • Performs other job related duties as assigned.

Qualifications:

Required Education and Experience:

  • High School Diploma or GED equivalent.

  • 2 plus years of relevant work experience.
    Required Licensure/Certification/Specialized Training:

  • Current licensure with the Texas State Board of Pharmacy.

  • National certification recognized by the Texas State Board of Pharmacy. National Healthcareer Association (NHA) or Pharmacy Technician Certification Board (PTCB)..
    Preferred Education and Experience:

  • Graduate from a pharmacy technology program.

  • 4 plus years of pharmacy experience in a hospital setting.
    Preferred Licensure/Certification:

  • National I.V. Certification accredited by the National Pharmacy Technician Association (NPTA).

This role collaborates with other members of the healthcare team to achieve case management department utilization, financial, quality, compliance and satisfaction outcomes for assigned patients. Typical Duties: * Conducts comprehensive psychosocial, functional, financial, healthcare support systems and discharge planning assessments. * Conducts patient, family, and/or representative interviews to assess needs and identify individualized discharge plans that takes into consideration the requests of the patient, family and/or representative. * Identifies patients who require assistance with eligibility determination for social programs and funding sources and generates referrals as appropriate * Develops an individualized discharge plan based on the patient's medical diagnosis, treatment plan, financial resources, psychosocial issues, outpatient healthcare support system, etc. * Educates patient, family, and/or representative regarding post-acute options and ensures that the patient's choices are incorporated into the discharge plan, when possible. * Participates in interdisciplinary rounds to facilitate communication and ensure that a comprehensive, coordinated plan of care is developed and implemented. * Provides brief supportive counseling to patients and families exhibiting complex family dynamics that directly impact patient care. * Serves as a resource person and provides counseling and intervention related to treatment and end-of-life decisions; advocates for patient and family empowerment and independence to make autonomous health care decisions. * Assists with the submission of guardianship applications, coordination of the process for orders of protective custody, and follow up on open APS and CPS cases. * Identifies the appropriate support system, options, and barriers associated with the discharge of the uninsured and undocumented patients while working with available support systems to transition the patient through the continuum of care. * Provides education and referral information regarding JPS and community resources. * Documents each component of the case management process and related activities in the electronic medical record system according to departmental policies and procedures. * Serves as an educational resource for physicians, nursing staff and others concerning case management strategies essential in meeting the organization's quality, utilization, financial and customer satisfaction objectives * Performs other related job duties as assigned Qualifications: Required Education and Experience: * Master's in Social Work from an accredited school of Social Work by the Council on Social Work Education. * 1- 3 plus years' social work experience, including internship work, performing any combination of: coordinating patient discharge and continuity of care planning; diagnosing underlying psychosocial needs; conducting comprehensive psychosocial assessment of patients; conducting initial screenings and evaluations of patients and families; providing or making referrals for individual, family, and group psychotherapy; and/or promoting patient navigation services. * Work experience must have been performed in an inpatient acute care, behavioral health, skilled nursing facility, long-term acute care, and/or outpatient setting such as home health, hospice, dialysis, etc. OR * Master's in Social Work from an accredited school of Social Work by the Council on Social Work Education. * 5 plus years' experience working with post-acute levels of care and community resources.
