Essential Job Functions

  • Establish plan of care reviewed by the Occupational Therapist.

  • Complete ADL assessment with guidance from the OTR within appropriate time frames.

  • Set ADL goals with measurable outcomes and appropriate time frames.

  • Initiate discussion with physician and other medical team members when appropriate to modify therapeutic program.

  • Discuss any changes in the treatment plan and/or patient response to treatment with Occupational Therapist in a prompt and professional manger.

  • Demonstrate appropriate use of various treatment modalities in patient care.

  • Complete screens as needed

  • Adhere to ethical and legal practice standards.

  • Abide by relevant ethical codes and standards of practice guidelines.

  • Abide by State and Federal laws and regulations, including state licensure laws.

  • Maintain records in accordance with department and system policies that are accurate, concise timely and legible.

  • Document therapy care in format and accordance with practice setting including: treatment provided, response to treatment, patient progress, education, family conferences, and discharge planning as appropriate.

  • Document education and training as well as patient and/or family level of achievement with training provided.

  • Educate all patient and families in a manner consistent with their level of understanding and therapeutic needs.

  • Consider patient and family goals in establishment with ADL goals.

  • Provide patient and family members with educational materials and programs as appropriate.

  • Document education and training as well as patient and/or family level of achievement with training provided.

  • Capture maximum revenue for quality services provided while maintaining established standards of care.

  • Maintain adherence to established efficiency productivity standards.

  • Complete daily charge entry accurately.

  • Assure proper authorization has been received prior to initiating treatment.

  • Collaborate with patients, facilities and other health care members to provide excellence in patient care.

  • Communicate regularly with other team members.

  • Participate actively in the ongoing development of new therapy programs and in the enhancement of established programs.

  • Attend appropriate internal or external education opportunities to promote growth and expansion of specific programs.

  • Implement a self-directed plan for professional development.

  • Initiate and participate in continuing education: lectures, video/audio conferences, written and online courses.

  • Develop and conduct in-service as appropriate to share information gained from external resources.

  • Maintain yearly record of continuing education via completion of Education Record.

  • Maintains responsibility for reporting accuracy of hours worked.

  • Staff schedule requires flexibility as they are dependent on patient care needs, which may include evening and weekend hours, as necessary to meet demand in their primary area or the needs across the system.

  • Travel to all Excela Health Home Care & Hospice service territories as patient census dictates, in order to provide quality necessary care when patient need is greatest.

  • Cognitive ability to function and concentrate on the essential and specialty functions of the job at a high level.

  • Achieves thoroughness and accuracy when accomplishing an essential or specialty function of the job.

  • Ability to think and concentrate for an entire shift.

Specialty Job Functions

  • Perform clinical responsibilities in a manner that positively affects care while achieving optimal outcomes.

  • Complete and document patient information/visits on a daily basis.

  • Respond to the plan of care established by the Occupational Therapist.

  • Discuss any changes in the treatment plan and/or patient response to treatment plan with Occupational Therapist.

  • Demonstrate appropriate use of various treatment modalities in patient care.

  • Capture maximum revenue for services provided while maintaining established standards of care.

  • Complete daily visits promptly and accurately.

  • Provide relevant information to insurance provider to insure proper authorization is secured prior to continued treatment as indicated.

  • Utilize supplies and equipment in a fiscally responsible manner.

  • Maintain a safe work environment.

  • Maximize utilization on non-direct patient care time.

  • Collaborate with patients, their families, DME providers and other team members to provide excellence in patient care.

  • Serve as a liaison for the patient with DME providers.

  • Participate in supervision of students as appropriate.

  • Participate actively in professional activities.

  • Participate in orientation of new Homecare personnel.

  • Other related duties as assigned.

Required Qualifications

  • Completion of a Certified Occupational Therapist Assistant program accredited by the AOTA, ACOTE.

  • Associates Degree, Certified Occupational Therapy Assistant.

  • Strong leadership ability, good organizational skills, independent thinking and decision-making ability, analytical problem solving skills, sound judgment, excellent oral and written communication skills, must be able to function in a team environment, and knowledge of legal aspects and liability of nursing practice.

Preferred Qualifications

  • Knowledge of computer applications, research experience, knowledge and/or certification in specialty treatment program (Low Vision, Lymphedema, Pediatrics, Hand Therapy, Women's Therapy, Occupational Medicine).

  • Minimum of 2 years' experience in the past 5 years in a Hospital, Skilled Nursing Facility, Home Health Agency or other relevant experience.

License, Certification & Clearances

  • Current licensure to practice in the State of Pennsylvania required.

  • Current Basic Life Support (BLS) approved curriculum of the American Heart Association (CPR and AED) program required.

  • Act 33 with renewal

  • Act 34 with renewal

  • Act 73 FBI Clearance

  • Valid Pennsylvania Driver's License (if out of state hire current Department of Motor Vehicle record report required and obtain PA Current Driver License within 30 days of hire).

Supervisory Responsibilities

  • This position has no direct supervisory responsibilities, but may serve as a coach and mentor for other positions in the department.

Position Type/Expected Hours of Work

  • Regular, consistent, on-site and timely attendance.

  • Incumbent will be scheduled based on operational need (rotate shifts, standby, on-call, etc.).

  • Travel may be expected locally between Excela Health locations.
