Responsibilities

Provides direct care to residents, while promoting the resident's highest level of functioning, as directed by management. Assists in maintaining the resident's environment in a safe and neat manner. Promotes quality services within community standards, State and Federal Regulations.

Receives and communicates report to management on resident status at the beginning and end of each shift and as needed. Notifies the management of any changes in resident medical condition/status. Provides personal care to residents per their individualized plan of care. Documents all pertinent information regarding care as assigned. Prepares residents for meals and assists to and from the dining areas. Assists with set up and feeding of meals. Assists residents with toilet and incontinent care. Makes incontinent checks at least every 2 hours and provides incontinent care as needed. Makes room checks and verifies resident location at least every 2 hours. Keeps resident's rooms neat and clean. Performs clinical procedures according to community policy. Performs vital signs, B/Ps, weights and other trained procedures as directed by management. Completes prevention and restorative care that includes but not limited to PROM, AROM, ambulation, transferring, bathing, grooming, and all other ADL care. Maintains a working knowledge of Community policy & procedure pertinent to role. Assists in turning, positioning, and repositioning of residents. Participates actively in Interdisciplinary Team Process including completing assessments and documentation as assigned. Attends care plan meetings as assigned. Acts as resident advocate. Communicates effectively with the residents, families, visitors, members of the health care team, and other disciplines. Actively participates in Community Quality Assurance Performance Improvement Program. Maintains education development to include attending ongoing education development to maintain compliance with State and Federal Regulations and current knowledge of resident care practices. Maintains resident confidentiality.

Qualifications

Education and ExperienceCertification of completing an approved Certified Nursing Assistant is required. Six months experience in long term care is preferred.

Certifications, Licenses, and Other Special RequirementsSuccessful completion of State C.N.A. course.

Management/Decision MakingMakes standard and routine decisions based on detailed guidelines with little independent judgment. Problems are generally solved with clear, detailed guidelines or by reporting them to a supervisor.

Knowledge and SkillsHas a basic level of a skill and can follow written or verbal instructions. Has basic technical skills to complete a few repetitive and well-defined duties. Has a basic knowledge of the organization in order to answer basic questions. Must have working knowledge of facility emergency procedures including fire and disaster drills. Ability to follow facility policies. Ability to maintain resident confidentiality and adhere to HIPAA policies.

Responsibilities Responsible for the day-to-day management of Brookdale's nursing staff within assigned community(s). Participates in developing, maintaining and updating written policies and procedures that govern the day-to-day functions of the assigned Nursing Unit. Ensures Nursing Department's Procedure Manuals contain all revisions and updates. Ensures that the staff assigned has been trained on any revisions or updates. Assists Director of Nursing (DON) in developing methods for coordinating nursing with other resident services to ensure the continuity of resident care. Ensures nursing personnel are in compliance with duties outlined in his/her job description and state/federal regulations. Assists the DON in developing current and comprehensive departmental job descriptions. Ensures that reference materials are current. Makes daily rounds to ensure all personnel are performing work assignments in accordance with acceptable nursing standards. Report findings to the DON. Reviews nursing notes for completeness; makes sure they are descriptive of the nursing care being provided and reflect the resident's response to the care. Monitors medication passes and treatment schedules to ensure medications are being administered as ordered and treatments are provided as scheduled. Authorizes the use of restraints when necessary and in accordance with the company's established policies and procedures. Performs administrative duties directed by DON, including completion of medical forms, reports, charting, etc. Participates in surveys and inspections made by authorized government agencies. Participates in the development, implementation, and updating of the departmental Quality Assurance Program. Participates in the development, implementation and updating of a comprehensive Residents' Discharge Plan. Serves as Discharge Planning Coordinator. In the absence of the DON, serves and participates in, and attends various committees as assigned. Assists the DON in evaluating and implementing recommendations from established committees. Assists the DON in determining departmental staffing needs with recruiting, training, etc.; ensures appropriate staffing levels are maintained. Assists in developing and implementing departmental organization structure. Participates in the development, planning, conducting and scheduling of in-service training classes that provide instruction on nursing protocols, and state/federal regulations; ensures a well-educated staff. Assists in the development and implementation of a nursing orientation program for new staff to the department. Participates in interviewing and selecting residents for admission. Informs staff of new admissions, their expected arrival, room assignment and other related information. Greets new residents upon arrival.
