CURRENT OPENINGS :

FT & PT - Day shift (6:30AM-3:00PM)

FT & PT - Evening shift (2:30PM-11:00PM)

PT, EOW - Night shift (10:30PM-7:00AM)

  • Current CNA certification required*

JOB SUMMARY

The purpose of this non-exempt position is to provide quality nursing care. This care will be performed in accordance with Mennonite Home Communities.

ESSENTIAL FUNCTIONS

  • Daily personal hygiene needs of the resident - i.e., bathing, dressing, hair care, mouth care, shaving, toileting, nail care.

  • Assist with daily and ongoing activity programs for all residents.

  • Addressing the needs of incontinent residents. Changing bed linens.

  • Lifting, turning, and transferring residents, as scheduled or as needed. Proper use of mechanical lifts requires 2 staff for all transfers.

  • Serving meals and snacks, feeding those who require help; providing fluids.

  • Perform restorative nursing functions as scheduled or as needed.

  • CNAs are scheduled for Resident care 24 hours a day every day.

  • Call bells will be answered in a timely manner.

  • Observing and reporting any change in resident's condition to LPN or RN using Stop and Watch in EMR.

  • Responsible to complete and document all areas of care in the EMR system. Weights/reweighs, BM records, restraint records (when applicable) input and output and meal percentages, ADLs, mood/behavior, toileting.

  • Responsible to adhere to standard/universal precautions and infection control prevention. Responsible to adhere to current PPE requirements.

  • Responsible to provide care in a manner that maintains resident's dignity and state of well-being.

  • The necessary skills to deal with confused, combative, disoriented residents calmly and professionally.

  • Complete annual Compliance and HIPAA training and exhibits behavior as set forth by the Code of Conduct in the performance of their duties. Reports any Compliance concerns timely.

  • Follows all local, state and federal regulations as they pertain to their position.

  • Adheres to MHC's Policies and Procedures covering Privacy and Security in Compliance with HIPAA regulations.

  • Responsible to safely transport residents in wheelchairs throughout MH.

OTHER FUNCTIONS

  • Responsible to maintain neat, clean resident rooms, whirlpool rooms and linen rooms.

  • Responsible to attend all required in-services Complete all required online education.

  • Responsible to carry out duties assigned by the Household Coordinator, Clinical Coordinator, or Care Coordinator.

  • Responsible to function as a household team member wherever assigned demonstrating a positive, helpful attitude; willing to assist at all times.

  • Responsible to function as part of the household which may include serving and cleaning up resident meals, light housekeeping duties, and participation in recreational activities.

  • Responsible to perform routine 1:1's with residents for ten minutes for each shift worked.

  • Deliver residents mail.

  • Attendance past twelve months- 0-3 call-ins=2, 3-6 call-ins=1, 6 or more=0

  • Late/Leave Early past twelve months- 0-4 occurrences=2, 4-8 occurrences=1, 8 or more=0

EDUCATION/EXPERIENCE/JOB RELATIONSHIPS

  • Membership on the Nurse Aide Registry, as monitored by the PA Department of Health. Certification must be active and in good standing.

  • The Nursing Assistant is responsible to run the RN or LPN in charge.

  • One year of experience in Long Term Care preferred.
