This position cares for our Veterans on the Skilled Care Unit of the facility.
This position works the PM shift, 2:45 PM to 11:15 PM, 32 hours per week. The PM shift receives shift differential pay of $2 more per hour during the week and $3 on the weekends.
Duties and Tasks
Work involves coordinating resident care in a household-type environment as part of a self-directed, cross-trained team at the North Dakota Veterans Home.
Coordination of the Household
Staff will be required to direct resident care and household needs including problem solving, staff schedule, scheduling daily activities and events.
Make good use of time when tasks are completed, spend time with residents or willingly assist other staff.
Demonstrate flexibility in work assignment
All resident care is provided according to each individual residents care plan and wishes
Attend and participate in facility committees and is involved with quality monitoring as appropriate
Complete Admissions and Discharges per Policy.
Provide care for deceased residents
Assist with resident movement activity (admissions, transfers and discharges).
Orientate new residents to the staff, residents and facility.
Record resident's clothing, valuables and other belongings. Mark clothing and personal items
Stock supplies - including housekeeping, and medical
Inventory supplies and place orders
Other duties as necessary.
Activities of Daily Living
Communicate with residents so that dignity is maintained.
Comply with the Verbal and Physical Abuse Policy and Resident Rights Policy. - Residents' problems and needs will be identified by use of communications skills, i.e., non-verbal cues, listening skills, reading 24 hour reports and observations.
Actively participate as a member of the Nursing Care Team so that resident care is enhanced.
Make good use of time when tasks are completed; spend time with residents or willingly assist other staff.
Demonstrate flexibility in work assignment.
Relate information to professional and paraprofessional staff.
All resident care is provided according to each individual residents care plan and wishes.
Use nursing assistant skills in response to resident needs and requests for care. Cooperate with all household staff to ensure that the resident's total regimen of care is maintained. Exercise good judgement in organizing work, setting priorities and balancing the interests of residents. Demonstrate flexibility to readily readjust priorities in response to resident needs.
Assist residents with daily care needs. This includes providing up to complete assist with all needs, including: tub bathing, showers, daily body washing, dressing and undressing which includes shoes, socks and ted hose or wraps, toileting, assisting with toileting needs including pads and briefs, colostomy/ostomy cares, applying lotions and creams, urinary catheter cares, nail care and shaving.
Maintain resident privacy at all times, i.e., close doors, use blankets, close curtains, discuss personal matters privately.
Assist residents with meals and nourishments. Feed residents who need total assistance and assist others as needed.
Make sure the prescribed plan of care for each resident is followed. Report changes to the Nurse.
Observe the resident's compliance care plan.
Encourage the resident to maintain their highest level of functioning.
Assist and instruct resident on carrying out current therapy program (i.e., stretching, range of motion, ambulation)
Observe and monitor resident's behavior. Report changes to Social Worker or Nurse.
Employees should understand behavioral intervention techniques.
Take vital signs and record (i.e., blood pressure, temp, pulse, weight, oxygen saturation)
Identifies, plans and incorporates resident's daily pleasures and interests into household life based upon recommendations from families and friends.
Plan, observes and participates in the household/facility activity calendar
Provides assistance to access activities and programs in/outside of the facility
Provides 1:1 interaction with residents based upon preferences
Encourage residents to achieve highest level of practical functioning based upon their interests and abilities
Provide adaptations for desired activities such as magnifying glass, headphones, and large print books
Perform a variety of miscellaneous tasks as requested such as shopping, running errands, assisting with correspondence, reading to the resident and distributing mail
Coordinate activity plans with Veterans Home staff and community resources
Maintain a safe, clean environment
Residents' living and/or treatment areas are left in clean, orderly fashion.
Report any needed building/equipment repairs and safety concerns.
Carry out infection control policies and procedures
Makes beds, change linens, set rooms in order and place call light within reach.
Perform day-to-day housekeeping functions in resident rooms and common areas
Clean resident rooms, closets and drawers
Clean and polish furnishings, fixtures, and ledges in resident rooms
Clean, wash, sanitize and/or polish bathroom fixtures
Clean window/mirrors in resident rooms and common areas
Clean floors to include: sweeping, dusting, damp/wet mopping, vacuuming
Remove dirt, dust, grease, film, etc. from surfaces using proper cleaning/disinfecting solutions
Discards waste/trash into proper containers and relines containers with plastic liners
Ensure that equipment, supplies, etc. are properly stored at all times when not used.
Distribute clean laundry, linen and garments

Chart on assistive devices (ted socks, sheepskins, egg crate mattress, isolation)
Chart when residents leave the facility for short periods of time or out for activities.

Complete Medical Records documentation accurately and to NDVH standards
Requires a high school diploma or GED. Must successfully complete nurse assistant (C.N.A.) certification
