Position Summary:

Responsible for providing personal patient care and related services for terminally ill patients in home or home-like settings.

Qualifications:

  • A high school graduate or equivalency is required.

  • The Hospice CNA must have a compassionate attitude toward the care of the sick, the ability to read, write, and carry out directions, and the maturity and ability to deal effectively with the demands of the job.

  • Preference is given to candidates with a CNA or HHA certificate from an approved program by the State of Florida or Alabama, as applicable, depending on assignment, with a curriculum that requires a minimum of 75 hours of training and a competency evaluation as described in the attached Federal Regulation 42 CFR 484.36, Condition of Participation: Home Health Aide services (a) Standard: Home Health Aide training. Candidates without the aforementioned certificate must successfully complete the Covenant Care Hospice CNA competency evaluation and in-service training program as described in the attached Federal Regulation 42 CFR 484.36, Condition of Participation: Home health aide services (b) Standard: Competency evaluation and in-service training. Completion of six (6) months experience as a Nurse's Aide in a hospital or medical surgical unit or six (6) months experience with a Home Health Agency or Hospice is preferred.

  • A valid Florida or Alabama driver's license, as applicable, depending on assignment, current automobile liability insurance and insurability under the company liability insurance are required.

  • If working in Alabama, must possess current proof of CPR training.

Job Relations:

Reports to: Director Branch Operations or Clinical Manager, as assigned for primary supervision.

Main Duties and Responsibilities

  • Competently performs all basic Hospice Aide skills as listed on the "New Employee and Annual Hospice Aide Skills List." This Hospice Aide Skills List must be completed annually and submitted with the annual performance evaluation.

  • Assists the patient with personal care which may include bathing, oral hygiene, hair care, routine skin care, catheter care, feeding, dressing, grooming, transfers from bed to chair, bedside commode, or wheelchair and helping with walking/ambulation.

  • Assists the patient/family with incidental household tasks essential to the patient's health needs such as light housekeeping, making and changing beds, dusting and vacuuming patient's room, dish washing, tidying kitchen, bathroom and bedroom, delivering needed supplies, and caregiver respite.

  • Assists the family in maintaining a healthy and safe environment.

  • Assists the patient/family unit with plans and preparation of nutritious meals and assists with occasional household chores when necessary.

  • May perform the following functions after instruction and supervision by the RN or Clinical Manager/Dir/Asst Dir Regional Operations/Branch Office Manager.

  • Assists with the use of devices geared to disability as aids to daily living.

  • Assists patient with prescribed exercises that the Home Health Aide has been taught by appropriate professional personnel.

  • Assists with preventive skin care. May reinforce non-sterile dressings.

  • Assists in providing light massage when approved by the primary nurse.

  • Records "input" and "output" if ordered.

  • Prepares diet as required by patient.

  • Takes oral or axillary temperatures as needed.

  • Reminds/Assists patient to take oral medications.

  • Encourages the patient to become as independent as possible and resume activities of daily living according to the patient's abilities and conditions.

  • Offers simple emotional and psychological support to the patient/family unit. Encourages patient mental alertness through his/her activities of interest.

  • Routinely collaborates with/reports to the IDG any change in the patient's mental or physical condition, or home situation. Coordinates with the primary nurse or designee to accomplish the required supervisory visits.

  • Participates in patient care conferences and participates in various learning modes and in-services to enrich personal and professional growth, and to maintain the required 12 hours (Florida) or 12 hours (Alabama) of in-service education per calendar year.

  • Accurately records and documents on forms and charts as designated by the Hospice Program.

  • Assists in the creation and maintenance of a climate that promotes growth and harmonious relationships within the Hospice staff.

  • For team staff only, attends morning reports.

  • Works in conjunction with scheduled shifts, on weekends as required.

  • Performs all other duties as directed by supervisor.

  • Required to comply with all Covenant Care Policies, Procedures and Personnel Policies, including those specifically addressing disaster preparedness and response.

  • Works effectively with volunteer services to identify patient/family needs that can be met by volunteers; offers volunteer services to patients and families, as needed, throughout their length of stay; requests volunteer services to meet identified needs; documents volunteer interventions in the plan of care.

  • Determines the patient is hospice appropriate, documents to the patient's terminality, and establishes and maintains the care plan for medically necessary assessments and services. This applies to determinations and consultations with members of the IDG, including the Attending Physician or Hospice physician, in all circumstances in which the level of care is to be assigned as short-term inpatient care or continuous care, and includes all actions required in Covenant Care's policies and procedures regarding the placement or removal of a patient from those categories of care.

  • As a member of the Interdisciplinary Group, is responsible for determining and documenting the level of care status of patients, and in particular, inpatient level of care, to ensure the patient is appropriate and meets the criteria for short-term care needed to manage pain and symptoms in an inpatient facility. This responsibility includes appropriate management and documentation with the patient's change in status either to short-term inpatient care or removed from that status, and all monitoring of the care during the inpatient period. This same requirement applies to management and documentation of patients who are placed on a continuous care level of care. Strict compliance with all rules and regulations, including Covenant Care's policies and procedures, is required.

Functions that the Hospice Aide will not perform:

  • Administer medications

  • Sterile dressings

  • Gastric lavage or tube feedings

  • Care of tracheostomy tube

  • Give injections

  • Make judgments or give advice on medical or nursing problems

  • Give any personal care not included by the nurse in the plan of care

  • Give douches or enemas

  • Irrigate colostomies

  • Apply hot/cold or wet/dry compresses

  • Take rectal temperatures

  • Adjust any medical equipment

Special Demands/Physical Requirements:

This position frequently requires medium to heavy lifting. Normally, such lifting will include, but is not limited to, assisting with the transfer/repositioning of the patient either in the home or the nursing facility. This position requires considerable time driving their motor vehicle in connection with performing the essential functions of the position.
