Position Summary:

Provides direct patient care services to the participant's under the direction and supervision of a nursing staff member, as assigned by the Program Director.

Essential Functions of the Job:

RESPONSIBLE TO: Nursing

RESPONSIBLE FOR: Direct participant care

AREAS OF RESPONSIBILITY:
1.Assists other staff in carrying out program assignments.
2.Performs and records routine technical tasks such as vital signs (temperature, blood pressure, pulse, and respiratory rate), collection of specimens, intake and output and mass.
3.Works in conjunction with other staff in relation to participant care, as assigned by the nursing staff.
4.Assist with admission, transfer, and/or discharge of patients, by transporting, lifting and positioning patients while following necessary safety precautions.
5.Reinforces selected patient teaching initiated by Registered Nurse.
6.Assist in maintaining patient and unit area in a clean and orderly manner.
7.Considers the special needs and behaviors of specific patient age groups
8.Is responsible for order and cleanliness of the program areas and equipment.
9.Carry out assigned task as deemed necessary by the Activity Director.
10. Performs home visits and assists with ADLs, transfers, companionship, etc.
11. Assists as companion to medical appointments, pharmacy, etc.
12. Provides personal care, such a toileting, bathing, eating, transferring, dressing or grooming under the supervision of the nurse and the Activity Director.
13. Assist the Activities Director in carrying out the activity program.
14. Assist other center staff in meal and snack serving. Performs meals counts as assigned by Day Center Manager.
15. Cleans-up and organizes program areas, including the cleaning of equipment.
16. Interact with patients and family
17. Transport patients and equipment as needed.
18. Keeps a record of care given.
19. Monitors patients and report any variances to normal to the nurse for further assessment.
20. Follows infectious disease precautions to prevent the spread of organisms.
21. Attends and actively participates in the in-take and assessment process as needed.
22. Attends center staff meetings and in-service educational training programs.

Additional Duties and Responsibilities:
1.Performs other duties as assigned.

Minimum Qualifications:
1.High school graduate or equivalent.
2.Nurse's Assistant California State Certification.
3.Current CPR Certification/Card
4.Bi-lingual (English and Spanish)
5.An interest and desire to work with the target population served by the center.
6.Required At least one year experience working with the frail elderly population

CONDITIONS OF EMPLOYMENT:

Physical clearance for reaching, bending, stooping, crouching, kneeling and grasping; have full range of motion; ability to lift 50 lbs; need to lift and transfer participants; T.B. clearance; must maintain current First Aid and CPR; fingerprint clearance.

VIEW JOBS 2/7/2020 12:00:00 AM 2020-05-07T00:00 Position Summary: The PACE Applications Analyst is an intermediate analyst position responsible for troubleshooting and coordinating the resolution of problems occurring with all business and clinical applications. He/she will work independently in managing application related service tickets, documenting findings and communicating status with the end-users. The Applications Analyst will support problem resolution, enhancement requests, and project tasks by building system components, testing, documenting workflows and related information, implementation support and maintenance of TruChart EMR along with other business and clinical applications as needed. He/she will create and evaluate processes, procedures, and workflows and will serve as the subject matter expert to users and other IT team members on the functionality of the organization's mission critical applications. The Application Analyst will be responsible for staff training, development of training guides for the different disciplines. The Applications Analyst will follow the software development lifecycle to manage development, testing, and deployment of changes. He/she will follow internal governance processes at all timesHe/she will be proactive in the investigation and application of best practices and have a good understanding of regulatory requirements related PACE. Essential Functions of the Job: 1. Ensures compliance with PACE regulations, assists to develop policies and procedures and assists with preparation for PACE audits for all TrueChart users. 2. Works closely with Quality Management to ensure TruChart user data meets quality and corporate compliance. 3. Participates in Project planning and meetings and works with PACE team as required. 4. Ensures Truchart, and computer access and new employee training and guides to all new and current employees. 5. Prepares and submits monthly reports to the VP of Senior Health Services. 6. Reviews changes and upcoming developments in software applicatins. 7. Creates documentation for supported systems and enhancements. 8. Ensures new employees are entered in TruChart and inactivate terminated employees. Provides new employee orientation and ongoing education. 9. Provide education to super users and coordinate end user education for patches, update, and upgrades including both impacts to functionality, clinical workflow, and business processes. 10. Technical troubleshooting for hardware issues – collaborating with local ITS – Desktop. 11. Oversees monitoring of error and efax failures as applicable to lab and pharmacy messaging. 12. Participate with testing TruChart updates. Coordinate testing of patches, updates and upgrades as well as reviewing / updating associated workflow and business processes. 13. Participate with distribution of TruChart notifications to appropriate staff such as System downtime notifications, program issues. 14. Review end user enhancement requests and submits those deemed appropriate to Electronic Health Record Project Manager. 15. Develops advanced knowledge of TruChart program functionality including understanding of encounter data. 16. Manage and maintain add/manage lists, tables, and forms as requested by staff. 17. Troubleshoot TruChart clinical issues and / or collaborate with PACE team as needed. Responsible for distribution of TruChart notifications. 18. Responsible for being the Point of Contact for any Truchart technical questions and information during site audits. 19. Adheres to SYHC and department's attendance and punctuality policies and practices. 20. Responsible for maintaining open communication with Eclusive, Capstone, Truchart, and SYH IT teams OTHER RESPONSIBILITIES 1. Technical Knowledge and Skills Required Performing the Job: Proficient in Microsoft applications and use of electronic health data systems, TruChart experience preferred. At a minimum level have good computer literacy including internet navigation and research skills.
