Position Responsibilities:

  • Participate as a member of the interdisciplinary team on an as needed basis.

  • Carry out plan of care activities within the CNA scope of practice as directed.

  • Assist and/or supervise participant's with activities of daily living in the center or when asked, in the participant's home to include (i.e., bathing, skin care, dressing, grooming, Foley care, decubitus preventive care, hair care, mouth care, eating, hand and food nail care, and toileting) as assigned per the care plan under the direction of the supervisor.

  • Create and participate in group(s) and one-on-one activities in accordance with daily activity calendar and as requested by the Activity/ Recreational Coordinator

  • Throughout the day, assist in maintaining clean and orderly day care center rooms, serving area, personal care areas and other areas of the center as directed.

  • Accompany enrollee to medical and other appointments as requested.

  • Assist transportation staff with the loading and unloading of participants from vehicles.

  • Serves meals and assist participants as needed with feeding & other needs as necessary. Reports participant food & fluid intake as needed into medical record and communicates any issues to the Activity Coordinator or Registered Dietician.

  • Performs skills/ services according to accepted procedures. These include taking vital signs, weight, height, collecting specimen, enema administration, performing Sure Step blood sugar test, measuring intake & output and assisting with participant group activities.

  • Reports any changes observed in the condition or behavior of participant to appropriate Eldercare personnel.

  • Maintains availability to assist during crisis or emergency situations.

  • Responds to participant call light/ request in a timely manner and follows through on participant need/request.

  • Accurately documents participant observation following Eldercare protocol.

  • Actively participates in Eldercare's Fall Prevention program.

  • Adheres to the principles of non-violent crisis intervention when dealing with aggressive participants. Assists in the de-escalation process.

  • Maintains therapeutic communication with participant. Gives re-direction and/ or reality re-orientation or validation as needed.

  • Attends departmental meetings and in-service programs.

  • Know and adhere to the philosophy and goals of Eldercare.

  • Act only within the scope of authority to practice.

  • Participate in and support Quality Initiative

Qualifications:

  • State of Florida Nursing Assistant Certification; Non-Violent Crisis Intervention Certificate within 1 year of employment. CPR and First Aid Certification required. First Aid may be completed in first 90 days.

  • High School Diploma or equivalent preferred

  • At least 1 year experience working with a frail or elderly population
