Responsibilities
We are looking for CNA's to join our Subacute care teams at Community Subacute & Transitional Care Center! As a CNA on our team, you'll provide direct and indirect care to our diverse patient population under the direction of an RN/LVN. As an empathetic caregiver, you will assist patients with daily living activities including, but not limited to: personal care, wound care, feeding, bathing and communicating with family.
When a patient needs ongoing care, subacute care steps in. Community Subacute & Transitional Care Center is a 106-bed facility providing inpatient care for chronic, subacute conditions such as coma or other complex illness that require 24-hour medical supervision.
Patients whose care requires ventilator, respiratory, wound, IV, hospice, rehabilitation services and/or general nursing can receive treatment that's consistent with an acute care center's effectiveness while in a comfortable long-term care setting.
Qualifications
High School Diploma, High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required
Certified Nursing Assistant training to obtain certification required
Incumbents hired into position prior to 05/14/2019 are grandfathered with their existing education degrees
Experience in a skilled nursing facility, respiratory subacute facility or acute hospital setting preferred
CNA - State of California Certified Nursing Assistant Certification required
BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
