The CNA is to provide daily care and services that support the care delivered to clients requiring in-home personal assistance and care, in accordance with the established certified nursing care procedures and as directed by your supervisor.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Ensure the Core Values of Patience, Dignity and Discretion are adhered to at all times.
Provide care as directed by the Case Manager to clients requiring in-home assistance with ADL's, AIDLs, transportation, light housekeeping and meal assistance.
Reports observations and other pertinent information related to the care of the client.
Documents information related to client care on agency forms or in electronic record.
Provides services that support the care delivered to the client.
Provides input in the formulation and evaluation of standards of care.
Participates in the development of client objectives.
Participates in the identification of staff educational needs.
Conducts self in a professional manner in compliance with agency policies.
Participates in the quality assessment and improvement process and activities.
Communicates client's progress to Case Manager; report condition changes immediately.
Assists clients to maintain their current level of function or to restore lost function of ADL's.
Maintain confidentiality of all pertinent client care information to ensure client's rights are protected.
Communicating correct and up to date information to supervisor or through the proper communication channels.
High School diploma or GED preferred.
Previous Nursing Assistant experience required.
Completion of a certification program for Nursing Assistants in this state required.
