The CNA is to provide daily care and services that support the care delivered to clients requiring in-home personal assistance and care, in accordance with the established certified nursing care procedures and as directed by your supervisor.


  • Ensure the Core Values of Patience, Dignity and Discretion are adhered to at all times.

  • Provide care as directed by the Case Manager to clients requiring in-home assistance with ADL's, AIDLs, transportation, light housekeeping and meal assistance.

  • Reports observations and other pertinent information related to the care of the client.

  • Documents information related to client care on agency forms or in electronic record.

  • Provides services that support the care delivered to the client.

  • Provides input in the formulation and evaluation of standards of care.

  • Participates in the development of client objectives.

  • Participates in the identification of staff educational needs.

  • Conducts self in a professional manner in compliance with agency policies.

  • Participates in the quality assessment and improvement process and activities.

  • Communicates client's progress to Case Manager; report condition changes immediately.

  • Assists clients to maintain their current level of function or to restore lost function of ADL's.

  • Maintain confidentiality of all pertinent client care information to ensure client's rights are protected.

  • Communicating correct and up to date information to supervisor or through the proper communication channels.

Minimum Qualifications:

  • High School diploma or GED preferred.

  • Previous Nursing Assistant experience required.

  • Completion of a certification program for Nursing Assistants in this state required.

He/She is responsible to the company for the efficient, safe, effective operation of the Pharmacy. * When a guest enters my area or department, my first and most important responsibility becomes to acknowledge, greet, and engage that guest * Responsible to our guests and for building a reputation for ultimate guest service * Responsible for handling of prescriptions, counting, labeling and reconstituting prescriptions * Maintain confidentiality when handling patient records. * Maintain guest charge and delivery records. * Mail out prescriptions * Responsible for calling physicians' offices for refills * Other duties as assigned by management * Must have high school diploma or equivalent * Must be registered with the State Board of Pharmacy * Must have earned and maintain PTCB certification through the designated PTCB training program and/or state required registration * Must maintain required continuing education and renew license as required by State Board * Flexible schedules, including evening and weekend hours required * Experience in retail pharmacy is a plus * Pharmacy team will train, but individual must have the ability to complete on the job training and exam which includes, but not limited to: * Guest service duties, efficient utilization of pharmacy technology (computer, email, etc.) * Prescription input and preparation * Pharmacy inventory control * Pharmacy related math * Must pose knowledge of Pharmacy related law * Excellent oral and written communication skills * Perform all other physical aspects of the job including bending, squatting, lifting, climbing, standing and walking for 8-10 hour period * Ability to function as a team member and get along with others * Ability to accept supervisory coaching related to performance, work habits and attitude.
