Position Summary

To provide personal care and supervision to residents in a manner conducive to their safety, comfort, security and the greatest degree of independence possible. A C.N.A. performs within the parameters of each residents care plan and daily schedule as determined by the interdisciplinary teams, and within the practice limitation of certification. Under onsite supervision of the Charge Nurse.

Education / Licensure Requirements:

Must possess current valid certification as a Certified Nursing Assistant in the State of Iowa. As evidenced by listing on Iowa's Nurse Aide Registry or documentation of successful completion of competency and skills tests from an approved testing agency.

Must complete ongoing in-service training requirements as well as completing assignments and return demonstrations as required.

Essential Job Functions:

  • Performs incidental housekeeping and maintenance task as may arise during the course of regular duties, in order to maintain a clean, safe, pleasant environment for residents, visitors and staff

  • Responsible to know and adhere to the care plan of each individual resident. This includes assisting in implementation of each residents care plan by performing personal cares such as dressing, feeding, toileting and grooming to assigned residents according to their care plans and daily schedules of care. This care will be provided in such a way as to promote safety, comfort, security and the greatest degree of independence possible

  • Observes and verbally reports to the charge nurse accurate and timely information concerning assigned residents, such as a change in medical condition of health status; lack of response or resistance to care; special problems; unusual incidents; residents needs or requests; behavioral or psychological concerns. C.N.A. should document the above in writing according to established procedures or as requested

  • Complies with the residents care plan in the performance of rehabilitation, range of motion, or periods of exercise as ordered by the physician, physical therapist, etc.

  • Documents all care and treatment provided according to established policies and procedures

  • Complies with safe lifting along with transferring and handling of residents per approved procedures. Assumes responsibility for the safety of residents under immediate supervision and those residents assigned at any time

  • Reports and documents resident incidents and injuries according to established procedures

  • Participates in information exchange with the staff from preceding and following shifts to assure continuity of care. Contributes knowledge about residents to assessment and care planning process and participates in meetings upon request

  • Performs resident care techniques and procedures according to certification instruction, in-service education and / or instruction.

  • Assists with admission, discharge and transfer of residents. Documents above according to established procedures

  • Responds to inappropriate or maladaptive behaviors by residents in a manner consistent with the care plan or established procedures, and which safeguards resident's rights

  • Listens to residents and family's questions and concerns and responds in a calm and concerned manner, controlling emotions. Reports above to charge nurse in a timely manner

  • Promotes positive public relations with residents, family members and guests

  • Provide quality nursing care to residents in an environment which promotes their rights, dignity, freedom of choice and their individuality as illustrated by the following:

  • Provide individualized attention, which encourages each resident's ability to maintain or attain the highest practical, physical, mental and psychosocial well-being.

  • Maintain the comfort, privacy and dignity of each resident in the deliver of services to them. Interacts with residents in a manner that displays warmth and promotes a caring environment.

  • Fully understand all aspects of residents' rights, including the right to be free of restraints and free of abuse. Responsible for promptly reporting to the charge nurse or administrative staff any incidents or evidence of resident abuse or violation of residents' rights.

  • Protect the personal belongings of each resident including eyeglasses, dentures, hearing aids, furnishings, jewelry, clothing, memorabilia, etc. Promptly report missing items according to established policy and participate in efforts to locate missing items.

  • Provide care that maintains each resident's skin integrity to prevent pressure ulcers in accordance with skin prevention protocol and company policies and procedures.

  • Must maintain skills necessary to safely and efficiently operate the following equipment and machines used in performance of this job:

  • Telephone

  • Mechanical lifts

  • Electronic scales

  • Shower equipment

  • Adjustable chairs / beds

  • Various other equipment and supplies

  • Read and follow Infection Control policies and procedures

  • Read and understand Material Safety Data Sheets for all supplies. Use supplies according to directions.

  • Treat all residents with kind and considerate care at all times

Tasks:

  • Assist resident in dressing and undressing as needed in clothing appropriate to the time of day and season of the year

  • Change bed linens

  • Assist resident in before and after meal routine (wash hands, face, transport to and from dining area, prepare bedside environment for in-room dining)

  • Provide prompt meal assistance (position tray for patient convenience, help with cutting foods, pouring liquids, etc)

  • Provide partial or total feeding assistance and encouragement as needed

  • Report appetite changes and request substitutions when necessary

  • Record observations on meal intake record

  • Provide adequate hydrations (pass ice and fresh drinking water, offer fluids frequently, record intake as directed, etc)

  • Provide between meal nourishment as assigned (distribute supplemental nourishment as assigned, provide assistance as needed, accurately records and report intake of nourishment)

  • Assist residents with all elimination needs (bedpan, urinal, bathroom assistance, etc)

  • Participate in facility incontinence program

  • Perform proper perineal care after each episode of incontinence

  • Perform catheter care per facility policy

  • Record bowel habits and urine outputs as directed

  • Collect urine or fecal specimens as directed

  • Reposition immobile residents at times and in a manner as instructed

  • Assist resident in transferring from bed, chair, wheelchair, toilet, etc., using appropriate equipment and following principles of body mechanics (determine number of persons for assist; use gait belt, mechanical lift, grab bar., etc.) as per resident care plan

  • Assist residents in exercise (active / passive range of motion, ambulation, etc.) as indicated by supervisor and individual care plan

  • Take and accurately record resident temperatures, pulse, respiration rate, weights, B/P's, and ankle measurements as directed

  • Answer call lights in your designated area within 15 minutes

  • Keep residents rooms neat and free of clutter

  • Follow assigned cleaning schedules

  • Advise supervisor or DON when supplies are low

  • Chart required information accurately and sign entries with name and title

  • Perform all other duties as assigned by supervisor
