The person in this position is a member of the Patient Centered Health Care Home team where each team member plays an integral role in providing patient-centered health care. S/he will provide healthcare management of women during the prenatal and postpartum periods, as well as in the areas of well-woman gynecology, primary care and family planning.
Supports the PCMH team in planning, developing, and implementing quality initiatives.
Practice and provide care in accordance with acceptable standards of Midwifery, the Clinical Practice Guidelines of KPHC, and other written policies and standing orders of KPHC.
Abide by a written Collaborative Agreement with University Health Partners OB/GYN physicians, who will provide medical supervision to CNM on an as-needed basis.
Provide for comprehensive care with the ability to consult and/or refer to other health care providers/services/agencies as necessary.
Order and interpret laboratory tests, studies and ancillary services.
Prescribe medical treatment and clinical drugs to patients.
Perform approved therapeutic procedures.
Maintain charting in accordance with legal and clinic standards.
Instruct and advise patients regarding preventive health care measures to promote physical and mental well-being and to promote early detection, diagnosis and management of medical conditions.
Participate in periodic chart review with supervising OB/GYN physician and clinical health care team to maintain continuity and consistency of care
Provide/promote "informed consent" and knowledgeable participation of clients in their care and in preventive care.
Maintain confidentiality of all patient information according to federal guidelines and regulations.
Assist in the resolution of complaints, requests and inquiries from patients.
Provide health education/consultation to clients, families, other professionals, and the community as appropriate.
Collaborate in planning and participate in teaching paraprofessionals, midwifery students, medical students, nursing students and other health care professionals.
Attend all staff and professional meetings as required.
Participate in health center and professional quality assurance plans/activities, data collection and analysis.
Participate in monthly peer review, support staff evaluation, case conferences, and other activities as required.
Assist in evaluating and revising practice policies and procedures.
Communicate problems, concerns and suggestions to the Medical Director and to other medical team providers verbally and in writing where appropriate.
CNMs who deliver at KMCWC will participate in Labor and Delivery staff conferences on morning starting call and on morning at the end of each call to ensure continuity of care.
Regular and reliable on-site attendance is required.
Perform other related duties as assigned
Observe safety and security procedures; determine appropriate actions beyond guidelines; report potentially unsafe conditions; use equipment and materials properly.
Minimum Qualification Requirements:
Participate in continuing education programs and maintain current BLS certification.
Current APRN licensure in the State of Hawaii
Certification by the AMCB.
Current Basic Life Support and Neonatal Resuscitation certification.
Maintain necessary certification for therapeutic procedures as needed.
CNMs who deliver babies will be required to maintain full Labor & Deliver privileges at KMCWC and/or other hospitals.
Must be eligible for credentialing with Quest and the majority of other third party payers that KPHC participates with.
Must be able to work with multi-ethnic and multi-cultural staff and clients.
Knowledge of and ability to apply professional medical principles, procedures, and techniques.
Must be able to use EMR effectively
Thorough knowledge of pharmacological agents used in patient treatment.
Effective verbal and written communication skills along with proper telephone etiquette.
Ability to assess and plan in a multi-cultural context
Ability to communicate effectively with multi-ethnic populations
