DUTIES & RESPONSIBILITIES:

  • Provide daily nursing care and services that support the care delivered to patients in the facility.

  • Deliver care services to patients including:

  • Assistance with bathing

  • Changing linens

  • Properly positioning patients

  • Offering hydration

  • Checking in on patients in the morning and night

  • Taking/ recording vital signs

  • May perform other similar duties as assigned by the nursing department when necessary

QUALIFICATIONS & REQUIREMENTS:

  • Must be a licensed CNA in good standing with the state (as required)

  • Able to react to emergency situations appropriately when required

  • Must be able to relate information concerning a resident's condition

  • Must be able to read, write, speak and understand the English Language

  • Ideal candidate must be a caring individual dedicated to providing high quality care to our residents/patients

The program offers a full range of health care services for individuals who are eligible for nursing home care but want to live at home. Saint Joseph PACE believes it is better for seniors with chronic care needs to be served in their home whenever possible.� Many PACE participants get most of their care from staff employed by the PACE organization in the PACE center. This position reports to the Day Center Supervisor. * Participate as a member of the interdisciplinary team in ongoing participant assessment and monitoring. Report on personal care services provided and the self-care abilities of participant, including changes noted in participant's condition. * As part of the health care team, provide personal care assistance that will maintain the independence of the participant to conduct activities of daily living, including toileting, showers, eating, and grooming.
