About the Job

  • Assisting patients with appropriate techniques to maintain personal hygiene and grooming, including shower, tub or bed baths and mouth, skin, nail and hair care.

  • Assisting patients in the use of toilet facilities, including bed pans and commodes.

  • Assisting patients in and out of bed using safe transfer techniques, including the use of gait belts and mechanical lifting equipment, when applicable.

  • Assisting patients with walking, including the use of walkers, canes and wheelchairs, when applicable.

  • Performing normal range of motion exercises and proper positioning.

  • Assisting with self-administered medications.

  • Assisting with meal preparation and feeding, when identified.

  • Grocery shopping when applicable.

  • Provide transportation needed (if identified on care plan).

  • Observing, reporting and documenting patient status and care provided.

  • Taking and recording of vital signs (temperature, pulse, and respirations) as identified.

  • Practice basic infection control procedures.

  • Documenting status of patient functioning and changes in patient status to be reported to the HHA's supervisor.

  • Submit accurate documentation and time sheets within established time parameters.

  • Maintaining a clean, safe and healthy environment in the patient's home including light house-keeping chores (ie; sweeping, vacuuming, mopping, dusting, dishes, bathroom cleanup), linen changes and personal laundry as identified on the care plan.

  • Recognizing emergencies and contacting emergency personnel and supervisor when necessary.

  • Understanding the physical, developmental and emotional needs of the populations served.

  • Maintaining adequate fluid and food intake for clients.

  • Attend at least 12 hours of training annually including mandatory OSHA and HIPAA training.

  • Treating the patient and his/her property with respect.

  • Comply with all Continuing Care Policies.

  • Comply with behavioral expectations of the Continuing Care Division.

  • Maintain courteous and effective interactions with colleagues and patients.

  • Demonstrate an understanding of the job description, performance expectations, and competency assessment.

  • Demonstrate a commitment toward meeting and exceeding the needs of our customers and consistently adheres to Customer Service standards.

  • Participate in departmental and/or interdepartmental quality improvement activities.

  • Participate in and successfully completes Mandatory Education.

  • Perform all other duties as needed or directed to meet the needs of the department.

Minimum Qualifications:


High School Diploma or Equivalent

Licensure, Certification & Registration:

The HHA shall have successfully completed a basic training program approved by the Mass. Association of Homemaker and Home Health Aides and possess original certificate evidence of such completion, or a student nurse matriculated in an approved school of nursing, having successfully completed one year of clinical training with supporting transcripts. Maintain a valid driver's license and vehicle registration and insurances.


One year CAN or CHHA experience preferred

About the Job The registered nurse plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. Part time - 32 hour day position Minimum Qualifications: Education: * Registered Nurse from a Graduate of National League for Nursing accredited school of nursing. * BSN, strongly preferred.
