Performs household services that are essential to the Home Health patients care and are necessary to prevent or postpone institutionalization.

1.1 Helps the patient with personal care, i.e. bed bath, assisting with tub bath, etc.

1.2 When appropriate, assist in meal preparation and feeding.

1.3 Assists patient with dressing.

1.4 Helps the patient in and out of bed, wheelchair, etc.

1.5 Performs light housekeeping tasks, i.e. laundering the patient's gown or pajamas and cleaning the patient's room.

Assists with certain treatments as ordered by the physician and approved and supervised by the appropriate personnel.

2.1 Performs simple dressing changes

2.2 Gives patient range of motion exercises.

2.3 Takes vital signs every visit.

Assist with the maintenance of clinical notes.

Exports clinical notes every working day by midnight.

3.1 Observes patient closely for signs and symptoms of change of behavior, condition of skin, color of skin, appetite, etc.

3.2 Reports any unusual condition or change in patient.

3.3 Reports all problems to the Case Manager or Supervisor.

3.4 Completes clinical notes in a timely manner in accordance with department standards.

Assumes responsibility for meeting standards of Nurse Aide.

4.1 Attends conferences, inservice education classes and other department activities.

4.2 Keeps informed and continually strives to improve her knowledge of skills.

4.3 Maintains professional appearance, attitude and performance.

4.4 Knows policies and procedures of Home Health Department regarding nurse aide practice, safety, health hazards, and personnel policies.

Demonstrates knowledge of special needs and behaviors of specific age groups including, but not limited to, adolescent, adult and geriatric.

6.1 Demonstrates knowledge of the changes associated with aging .

6.2 Demonstrates ability to provide care based upon physical, psychosocial, cultural, spiritual and other related factors.

Performs other duties as assigned by the Supervisors, Managers.

Education: Must have a high school diploma or GED. Complete appropriate hours and pass test required for the position.

Experience: Six months experience working with people and an interest in working with people.

Certifications: Nursing Assistant or Home Health Aide certification. American Heart Association Basic Life Support (BLS) Required.


Preferred Experience: 2 years of experience in Home Health setting.

Preferred Certifications: Home Health Aide or Long Term Care Aide Certification.

Essential Functions: The Health Information Specialist responsibilities include, but are not limited to, the following: * Accurately and efficiently processes requests for health information access and disclosure in compliance with HIPAA and other federal and state statutes as well as organizational policies and standard operating procedures * Logs and tracks all requests for access and disclosure of information in the information system in accordance with policies and standard operating procedures * Monitors and manages multiple sources for requests for information such as work queues, patient portal, fax, mail, and walk-ups * Reviews health information within multiple source systems, locations, and formats; discerns whether information is part of the legal medical record, billing record, or designated record set and understands access rights and requirements for each; and comprehensively and concisely identifies and discloses information requested and/or authorized * Ensures that medical records are complete prior to disclosure and works with HIM Operations to facilitate completion of the records as necessary * Reviews medical records for legally protected and sensitive information and obtains additional authorization, approval, or documentation when necessary * Performs activities related to the patient portal such as activation, proxy access, and troubleshooting and other health information exchange activities to facilitate electronic exchange of PHI * Assists with processing of medical record amendment, accounting of disclosure, and/or release restriction requests * Provides knowledgeable and competent customer service and uses various tools and information systems to facilitate the process; serves as a health information ambassador and patient liaison by ensuring rights to health information including privacy as well as right of access while encouraging patient engagement in healthcare by providing easy and timely access * Serves as an expert resource for privacy and disclosure matters for HIM employees and other department staff Accountability: The Health Information Specialist reports to the Manager for Health Information.
