Position Summary
General Summary/Responsibilities:
The Home Care Aide is a paraprofessional member of the health care team who works under the supervision of a licensed nurse to perform basic nursing tasks as assigned.

§ Must have completed the ninth grade and be able to read, write and follow written instructions and document care given.
§ Passport personal staff must have obtained a high school diploma or completed GED equivalent
§ ESP or Elderly Services Program staff must have obtained a high school diploma or completed GED equivalent.

Experience
Minimum Level of Experience Required: 1 - 3 years of job-related experience
Prior job title or occupational experience: Must have completed a basic aide training program and possess a Certificate of Course Completion or have one year hospital or skilled nursing home experience.
The basic aide training program must meet OBRA 87 requirements. Medicare, Medicaid, and Waiver aides must complete a two part competency evaluation. (Part 1 written, Part 2 demonstration of skills.) The score on the written portion must be at least 70% or greater. Private pay and Passport aides

Knowledge/Skills
§ Must also complete the written portion of the competency evaluation with a score of 70% or greater.
§ Agency must complete competencies.
Must display good physical and emotional health and be capable of stooping, bending and lifting up to 50 lbs. Should demonstrate good personal hygiene.
Must be self-directed with the ability to work with little direct supervision and have the maturity necessary for establishing and maintaining a good work relationship with the patient, the patient's family and all other office personnel.
Must have a sincere interest in and sympathetic attitude toward caring for the sick and disabled at home.
Must meet all agency personnel policies and have reliable transportation.

Organizational Standards
§ Performs total care or assists patients in all activities of daily living and personal care such as, but not limited to, complete or partial bath, oral hygiene, hair care, nail care, assistance with dressing and toileting and elimination.
§ Demonstrates knowledge of skin care including prevention, monitoring and reporting.
§ Takes and records accurately vital signs (temperature, pulse, respirations) and reports changes to the RN.
§ Assists patients with transfers, ambulation, exercises and use of assistive devices.
§ Demonstrates knowledge of normal R.O.M. and positioning.
§ Meets the safety needs of the patient and uses equipment safely and properly (i.e., assistive devices, oxygen and side rails). Verbalizes and demonstrates knowledge of home safety and safely assisting patients with medications.
§ Follows all agency policies regarding infection control and employee safety.
§ Recognizes patient's needs and attends to their requests in a prompt and organized manner.
§ Maintains a neat and orderly patient environment.
§ Utilizes appropriate communication skills with the patient, the patient's family and all agency personnel.
§ Prepares nutritious meals within the patient's diet and assists patient with eating when necessary. Demonstrates adequate knowledge of therapeutic intake and output.
§ Completes accurate records and carries out all assignments as requested. Is responsible for maintaining minimum weekend on-call, holiday or other requirements established by the department.
§ Demonstrates respect for patient property and maintains patient confidentiality.
§ Observes basic elements of body functioning and reports changes to the RN.
§ Recognizes emergencies and demonstrates knowledge of emergency procedures.
§ Demonstrates reliability and is able to arrive on time to patient assignments, dressed and groomed in a professional manner.
§ Performs only under the supervision of an RN who has provided written instruction for patient care (Care Plan). Participates in ongoing development of the Plan of Treatment.
§ Understands safety, equipment management and infection control.
§ JOB LIMITATIONS:
§ May not receive orders from a physician.
§ Will not administer medications or pre-fill insulin syringes, enemas (other than Fleet), tube feedings or rectal suppositories.
§ May not perform any sterile procedures or perform any intravenous procedures.
§ May not perform procedures requiring the knowledge, training and skill of a licensed nurse.
§ Is not authorized to perform non-patient related housework without prior authorization from supervisor.
