Maine Medical Partners has an opening for a Diabetes Educator-Registered Nurse. The candidate joining our practice will support patients living with diabetes through 1-1 patient visits and on-going classes. Our programs are recognized by the American Diabetes Association and emphasize a patient-coaching and team-based approach. Topics include nutrition counseling, diabetes self-management, and training on innovative technology: building alternative class modalities delivering methods to include on demand and web-based technologies.

An RN license and 2 years of nursing experience are required for this position. If you are not currently credentialed as a Diabetes Educator, we will work with you to obtain the credential within your first two years.

This position offers flexible scheduling, working 32, 36 or 40 hours. Shifts can be 4-8s, 4-9s or 4-10s during the Monday-Friday work week. Let us know what type of schedule you're seeking!

  • Position Summary

  • Under general supervision of the Clinical Manager and Medical Director, responsible for nursing assessments, as well as assessing diabetes educational needs. Plans, implements and evaluates nursing care and diabetes education for patients with diabetes and their family members in both individual and group settings.

  • Required Minimum Knowledge, Skills, and Abilities (KSAs)

  • Education: Completion of Bachelor's degree in nursing from an accredited school of nursing.

  • License/Certifications: Current Maine State Registered Nurse license. Current BLS certification. Currently certified or in process of achieving certification as a diabetes educator.

  • Experience: 2 years' experience as a Registered Nurse.

  • Knowledge of diabetes disease process, lifestyle recommendations, pharmaceutical therapies and self-management practices and provides information and education in accordance with the American Diabetes Association Standards of Care.

  • Knowledge of patient education and community health theories and practices related to disease prevention, behavioral change and methods of instruction.

  • Knowledge of nursing theory, practice and education sufficient to coordinate care for patients within the clinic setting.

  • Analytical abilities necessary to evaluate and revise educational programs based upon experience and patient performance.

  • Ability to communicate effectively both orally and in writing.

  • Interpersonal skills necessary to conduct formal education programs to counsel patients and family members.

  • Ability to maintain confidentiality.

  • Proficient computer skills.
