SUBFUNCTION DEFINITION: Responsible for a variety of training and development duties throughout the hospital. Much of the work focuses on ensuring that nurses have the skills and training to succeed in their individual units.

PURPOSE OF POSITION: To function as a professional nurse or provide medical nutrition therapy and to apply in-depth knowledge of diabetes in the care of infant; children; and adolescent patients and their families.

To provide clinical management and diabetes education for hospitalized pediatric patients; and assistance with clinical administrative issues within the diabetes practice. Provide education and consultation on the management and/or prevention of diabetes. To rapidly recognize and anticipate subtle changes based on past experiences; to coordinate and communicate with appropriate resources as needed.

To serve as a department-based role model and resource for policies; procedures and nursing care. To assist others in setting nursing care and department priorities; keeping in mind the education of self and others while maintaining a cohesive environment.

REPRESENTATIVE RESPONSIBILITIES

  • Care Integration

Works as an effective member of a multi-disciplinary pediatric diabetes care team (MDs, nurse practitioners, nurses, dietitians, and social worker), and working with children with diabetes. Provide education to school nurses, school personnel and other care providers.

Maximize the use of interdisciplinary team resources to enable chronic disease management for patients with diabetes. The incumbent will focus on diabetes care, and preventative treatment. Provide direct patient management, education, and discharge planning to children/families with new onset and ongoing management of the condition post-diagnosis in the outpatient setting.

Works collaboratively with the treating provider to manage and monitor treatment and the response to therapy. Provides assistance to the patient and family in obtaining necessary prescriptions for supplies and medication. Provides education to hospital staff through consultation or in-service education.

Precepts new staff. Educate and train health care professionals involved in caring for children with diabetes.

  • Patient and Family Education

Provides pre-diabetes and diabetes instruction in group and individual sessions. Obtains individual patient assessment.

Identifies patient/family problems or learning barriers including physical, cognitive, environmental, language/cultural, financial, psychosocial and spiritual and provides appropriate referral as necessary. Establishes individualized learning goals, objectives and educational plan in collaboration with the patient and the patient's caregiver(s).Evaluates patient upon completion of educational plan and provides remediation/plan modification as indicated and in consultation with diabetes team members. Develop educational materials on diabetes self- management based on needs of families or requests by staff in accordance with Clinical Content Committee guidelines, including materials on diabetes self- management, insulin adjustment, use of insulin pumps, and effective educational strategies for families with diabetes on patients from diverse ethnicities for both type 1 and type 2 Diabetes Mellitus. Stress the importance of properly managing diabetes to lower the risk of complications and reduce the chances of hospitalization.

  • Outreach

Participates in community diabetes programs/outreach activities. Serve on diabetes advisory boards, committees, to promote diabetes community events and services.

Supports quality standards and initiatives set by department. Actively participate and accept responsibility for special projects such as speaker requests, and diabetes family programs/events.

  • Clinical Practice

Assess DSME&S of patient and family including use of prescribed technology. Provides diabetes self-management education and family support for patients with diabetes in the inpatient, outpatient setting.

Provide both individualized and group DSME&S family education. Work with patients diagnosed with diabetes and their families, to facilitate improving self-management age specific goals for all patients. Work with specialized endocrine populations to facilitate improving self-management age specific goals for all patients.

Provides education that reflects the National Standards for Diabetes Self-Management Education. Determines appropriate educational plan for family and patient based on learning assessment with consideration for growth and development, psychosocial, and emotional factors. Demonstrates expert knowledge of diabetes by providing current, fact-based information.

Coordinate education to maximize efficiency, timely and safe discharge from inpatient or outpatient services. Maximize efforts to provide value based, cost effective diabetes education.

  • Program Support

Supports quality standards and initiatives set by department. Participates in practice changes as a result of performance improvement activities.

Evaluates effectiveness of educational services provided by measuring attainment of learning objectives. Conducts a follow-up assessment upon completion of program services, using outcome measures. Documents assessment data, educational plan, educational services provided and evaluation results in each participant's educational records.

Sustain a working understanding of regulatory requirements and accreditation standards. Support leadership to meet and sustain the regulatory compliance efforts of the department and hospital. Seeks professional growth or opportunities to advance care and knowledge including but not limited to diabetes technology, quality improvement work, chronic care, behavioral change, diabetes self-management education, and growth and development.

EDUCATION/EXPERIENCE

Required:

  • Bachelor's degree in a related field
  • 1 year of work experience in a related job discipline
  • OH Registered Dietician license OR OH Registered Nurse license
