Certificated Teacher: Substitute

INTERNAL APPLICANTS

Conditions of Employment

• Are you a citizen or do you have a visa which permits you to work in the United States?
• Are you physically able to perform the duties of the job for which you are applying?
• Are you willing to submit to a criminal history background check? The record check shall include a fingerprint check with the costs to be borne by the employee.
• IF YOU ARE SELECTED FOR A POSITION with Meridian School District No. 505, you will be expected to meet the following requirements as conditions of your employment: 1. Successful WSP and FBI background clearance check; 2. Washington State Sexual Misconduct Disclosure Release Form; 3. I-9 Employment Eligibility (citizen or legal alien status); 4. W-4 Form; 5. Washington State Department of Retirement Status Form; 6. HIV/Bloodborne Pathogen and Hazardous Materials training (verification of prior training or the district will provide); 8. Hepatitis B Training (if required); 9. Social Security Card and valid Driver's License (for identification upon hiring), to be copied by Personnel Office; BY SELECTING "YES", YOU AGREE TO PROVIDE/COMPLETE THESE REQUIRED ITEMS.
• I authorize Meridian School District No. 505 to make any investigation of any personal, educational, vocational, or employment history. I further authorize any former employer, person, firm, corporation, educational or vocational institution or government agency to provide the District with information they have regarding me. I hereby release and discharge the District and those who provide information from any and all liability as a result of furnishing and receiving this information.
• I certify that answers given herin are true and complete to the best of my knowledge. In the event of employment, I understand that false or misleading information given in my application or interview(s) may result in discharge. I understand that also I am required to abide by all policies and regulations of the District.

• Within the last seven years, have you ever plead guilty, been convicted, fined, imprisoned, or placed on probation for violation of any law, police regulation, ordinance, excluding minor traffic violations? A conviction records will not necessarily bar you from employment.

• If you answered "YES" to Question No. 1, please provide an explanation below.

• Within the last ten years, have you ever been discharged or forced to resign for misconduct or unsatisfactory service from any position?

• If you answered "YES" to Question No. 3, please provide an explanation below.

• Have you previously been employed by Meridian School District No. 505?

EXTERNAL APPLICANTS
