Overview

The Outpatient Rehab Central Patient Services Coordinator is responsible for assisting patients and providers in scheduling and registering outpatient therapy appointments for multiple disciplines and specialty areas at multiple locations.

This is a full time position that is authorized for 80 hours per two week pay period located at our Kasota office in Minneapolis, MN. It is benefit and PTO eligible.

Responsibilities/Job Description

  • Schedules therapy appointments for multiple sites and disciplines

  • Produces efficient and accurate schedules

  • Records and updates schedule changes accurately

  • Communicates changes to all involved staff in a timely manner

  • Offers and sends confirmation letters for patient appointments

  • Coordinates scheduling with outside vendors

  • Radiology and durable medical equipment companies

  • Provides educational information to patients regarding therapy and specialty programs including

  • Cancer rehab, vestibular rehab, cardiac rehab, pulmonary rehab, lymphedema, video-swallow evaluations, breast wellness, wheelchair evaluations, weight management, pediatrics, feeding clinic

  • Performs appropriate registration functions, including initial entry of demographic information, insurance information, referring physician data and referral record creation.

  • Physician orders:

  • Reviews and verifies all physician orders for complete order requirements

  • Requests and obtains physician orders when an order received is not in compliance/complete

  • Tracks non-compliant orders to ensure completed prior to evaluation date

  • Requests and obtains medical records for safe patient care when necessary

  • Obtains and verifies physician orders and medical records as needed.

  • Maintains working knowledge of multiple specialty services and therapy disciplines, clinic locations, directions, and parking information.

  • Provides directions to patients for up to 20+ sites along with landmarks for directions and department hours and locations

  • Provides parking information for all sites

  • Responsible for answering multiple telephone lines and responding timely and accurately to the caller's needs.

  • Participates actively in department problem solving and team meetings

  • Requests guidance and approval as needed.

  • Seeks out solutions to time conflicts directly through supervisors and directors

Duties include; checking patients, collecting co-pays, answering incoming phone calls for clinic appointments and other communications with staff or providers. In addition verifies patient information to include insurance verification and updates any changes in the electronic health record. Responsible for checking patients out after the visit and making any necessary follow-up appointments. The PSC will monitor activities in the waiting room to ensure a safe and patient friendly environment, communicating to patients when providers or clinics are running late. RESPONSIBILITIES * Utilizing the electronic health record accurately and efficiently registers and schedules patients * Perform the following functions: * Check patient in * Collect co-payments * Schedule and reschedule appointments * Verify insurance, updating patient information in the scheduling system or universal data set as part of the PAR process * Check patient out and schedule follow-up appointments within the clinic or in other hospital departments * Provide patient services with excellent customer service * Responsible for handling patient financial transactions: * Cash Box * Cash Collections * Credit/ Debit card transactions * Daily reconciliation reports * Provides basic technical support and maintenance of computer/ fax/ copier and hardware * Performs general receptionist duties * Provides customer service for patients, visitors, guests, and the interdisciplinary health care team * Coordinates obtaining and maintaining designated supplies and inventory for the clinic * Promotes the organization and assists in maintaining clinic efficiency * Ensures patients are checked-in in a timely and friendly manner * Ensures personal workspace and patient waiting areas are kept neat and sanitized * Perform other duties as assigned INPATIENT/ NURSING SUMMARY Under general supervision, greets, responds to and provides customer service to patients and their families, visitors, guests and the interdisciplinary health care team while supporting a patient and family centered healthcare environment. Facilitates the nursing unit activities generated by accurately and efficiently performing the non-clinical tasks to support patient care and service. Performs general receptionist and clerical duties for the unit as well as coordinates unit communications. RESPONSIBILITIES * Functions as a central point of communication for the unit * Provides customer service for patients, visitors, guests, and the interdisciplinary health care team * Promotes organization and smooth functioning of unit and assists in maintaining unit efficiency * Performs general receptionist duties * Coordinates patient admissions, transfer and discharge process within EPIC and direct patient service * Coordinates obtaining and maintaining designated supplies, equipment, and inventory for the unit * Provides basic technical support and maintenance of computer functions * Serves as a resource for health care team members, to assist them in navigating the electronic health record * Monitors, facilitates, and acts on orders RADIOLOGY SUMMARY The Patient Services Coordinator coordinates the scheduling and check-in of patients arriving to the Radiology Department. They perform general receptionist and clerical duties to facilitate the efficient functioning of the department. RESPONSIBLITIES * Serves as first point of contact for patients arriving to the department * Utilizes good communication and listening skills * Accurately transcribes/enters physician orders according to hospital policy and procedures; or finds physician orders on EPIC * Schedules patients for Mammography, Ultrasound, CT, MRI, General Radiology, Fluoroscopy, Interventional Radiology, IR Clinic, and Vein Clinic procedures * Evaluates next day's schedule to be sure we have order if not scheduled in EPIC * Performs appointment reminder phone calls to outpatients * Coordinates care team needed to perform scheduled radiographic procedures: interpreter services, anesthesia, Pediatric Intensivist, Radiologist, etc.) * Arranges patient transportation to and from the department * Prepares ID and allergy bands for patients with scheduled procedures and verifies patient information and performs Check 2 when patients arrive * Ensures that verification for reimbursement (MSPQ) is complete and Advanced Beneficiary Notices (ABN) are completed when alerted * Directs patients to changing areas and provides changing instructions to patient prior to the exam * Alerts technologist staff when OR cases are called * Provides excellent customer service for all HCMC patients, visitors and staff * Serves as a resource for new staff, peers, and other members of the health care team * Practices proper telephone etiquette by handling calls in a prompt, respectful and professional manner * Straightens and cleans up waiting room and front desk daily * Brings issues regarding processes, equipment, etc. to supervisor's attention when it happens * Collects co-payments, records, balances and closes out the cash drawer daily * Assists with other duties as assigned but only when trained to do so AMBULATORY QUALIFICATIONS Minimum Qualifications: * One year office clerical experience * OR- * An approved equivalent combination of education and experience * AND- * Valid driver's license and regular access to an insured vehicle Preferred Qualifications: * Bilingual Spanish/ English * Basic computer skills including experience with an electronic health record, Epic experience * One year of related experience in a similar (clinic/ ambulatory) healthcare environment Knowledge/ Skills/ Abilities: * Ambulatory Float Pool positions require ability to travel between clinic sites in an expeditious manner (i.e., candidates should not be dependent upon public transit schedules) * Proficient in Microsoft Office * Ability to successfully handle multiple tasks and duties simultaneously in response to patient/family and healthcare team needs * Ability to read and follow written instructions; understand and communicate verbal and written information * Ability to work cohesively and respectfully with a diverse work force and patient population * Ability to meet quality and productivity standards, including accuracy in patient registrations and scheduling as well as customer service expectations INPATIENT/ NURSING QUALIFICATIONS Minimum Qualifications: * One year office clerical experience * OR- * Approved education * OR- * An approved equivalent combination of education and experience Preferred Qualifications: * Basic computer skills including experience with an electronic health record, Epic experience * One year of related experience in a similar healthcare (hospital/ inpatient) environment Knowledge/ Skills/ Responsibilities: * Proficient in Microsoft Office * Ability to work cohesively and respectfully with a diverse workforce and patient population in a patient and family centered work environment * Knowledge of medical terminology * Ability to successfully handle multiple tasks and duties. simultaneously in response to patient/ family and healthcare team needs * Ability to effectively communicate information verbally and in writing as well as the ability to read and follow verbal and written instructions * Ability to meet quality and productivity standards, including accuracy in patient registrations and scheduling as well as customer service expectations RADIOLOGY QUALIFICATIONS Minimum Qualifications: * One year office clerical experience * OR- * Approved education * OR- * An approved equivalent combination of education and experience Hennepin County Medical Center Minneapolis MN
