SUMMARY:  Assists adults with developmental disabilities in their daily activities and needs in a residential setting. Observe residents, record, and report any physical or emotional irregularities. Assist with personal hygiene of residents, as needed. Assist with and supervise basic self-help skills. Give guidance to residents regarding desirable health, hygiene, and appropriate behavior. Chaperone, participate in, and initiate social activities and discussions with family and group. Participate in and oversee basic housekeeping duties in the home and surrounding premises. Transport residents to medical and other appointments or activities, as needed.

Job Type: Full-Time

Rate: $20.50 - $21.50 Hourly

Schedule: Monday- Thursday: 6am- 9am & 3pm- 9pm, Friday: 6am- 9am, Sunday: 3pm- 9pm (Overtime Built in)

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:

1.       Implements Individual Program Plan consistent with principles of normalization and the philosophy of New Horizons.

2.       Maintains anecdotal Progress Notes and daily charts current and complete including any progress made on Individual Program Plan and any other information pertinent to the health and well-being of each individual resident.

3.       Provides day-to-day training and supervision of residents. In addition, remains on premises or, with the knowledge and approval of the Administrator, arranges for an approved person to be present when any of the residents are home or expected home.

4.       Organizes day-to-day household management, which shall include: menus, food shopping, preparation and nutrition, housekeeping, laundry, house maintenance needs, and resident transportation.

5.       Serves as the facility’s Petty Cash Custodian, Resident Personal and Incidental (P&I) Funds Custodian, P-Card custodian, and gas card custodian while adhering to all agency policies and procedures as they relate to handling and securing funds.

6.       Administers medications to residents as prescribed by the resident’s physician. Ensures over the counter medications are adequately stocked in the medication cart.

7.       Monitors and coordinates social/recreational activities programs of residents. Coordinates corresponding vehicle use.

8.       Transports and accompanies residents to recreational outings (ie movies, musicals, parks) and medical appointments using the agency’s 8-passenger van.

9.       Participates in initial and ongoing training.

10.   Responsible for reporting immediately to the Administrator any unusual incidents that might be harmful to the health, safety and/or welfare of the residents, staff, and visitors.

11.   Communicates all resident medical problems to the administrator and recommends needed equipment and supplies for the program to the Administrator

12.   Maintains adequate fire, health, safety and welfare standards of the home and its surrounding areas according to Title 22. Conducts and documents monthly fire drills.

13.   Conducts monthly facility inspections to assure that all standards meet the standard of Community Care Licensing and HUD specifications.

14.   Responsible for ensuring facility vehicle maintains compliance with Cal Trans requirements which shall include ensuring that vehicle inspection sheets and mileage logs are completed and submitted monthly.

15.   Attends agency meetings as assigned by Administrator and/or Director of Residential Services.

16.   Attends monthly Home Manager meetings and other mandatory agency meetings as requested by the Administrator and or Director of Residential Services.

17.   Reinforces appropriate dress and social behavior in the community.

18.   Responsible for other duties that may be assigned from time to time.

OTHER DUTIES AND RESPONSIBILITIES include the following:

1.       Attends meetings as directed by the Community Care Administrator.

2.       Supports the Agency’s mission and partners with others to accomplish the mission.

3.       Maintains the highest professional and ethical standards at all times.

4.       Performs other duties as assigned.

SUPERVISORY RESPONSIBILITIES:  This job has no supervisory responsibilities.

Requirements

QUALIFICATIONS:  To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Must have a valid California Driver License and good driving record. Must have current First Aid Certification. Must be willing to work weekends. Must have current First Aid certification, valid California Driver License and a good driving record. Must pass a criminal background check. Must have a current physical and PPD test.

EDUCATION and/or EXPERIENCE:  High School Graduate or equivalent preferred. College experience in psychology, social work, special education, or home economics/child development desirable. Experience with individuals with developmental disabilities recommended.

LANGUAGE SKILLS:  Ability to read in English a limited number of two-and three-syllable words and to recognize similarities and differences between words and between series of numbers. Ability to print and speak in English simple sentences.
