Main Function: To enable and assist residents, families and significant others in adjusting to illness, losses, and appropriate placement.

RequirementsBachelors Degree in Social Work or a related field required.

Experience in health care preferred.

Position requires flexibilty to work occasionally on weekends and to provide coverage for absences.

Learn more about this agency Responsibilities The incumbent will work on the VA Caregiver Support Line at the Canandaigua VA Medical Center. The incumbent will work with a team of social workers to answer a wide variety of support line calls from caregivers, Veterans, community partners and families. The incumbent will provide information on Public Law 111-163, Caregivers and Veterans Omnibus Health Services Act of 2010. The incumbent will provide caregivers of Veterans psycho-education and emotional support around the challenges of being a caregiver. Incumbent will deliver information about and connect callers to VA and community resources, including referrals to local VA Caregiver Support Coordinators. Incumbent will work with caregivers and Veterans who are experiencing a variety of mental health, medical and social problems, utilizing telephone counseling and therapeutic skills. Incumbent will work on special caregiver support assignments and projects when not engaged in telephone work. Such assignments and projects include, but are not limited to, facilitation and curriculum development of Telephone Education Groups and quality assurance endeavors. GS-9 social workers provide psychosocial services in the assigned area under supervision. GS-11 Social Worker: Experience, Education and Licensure: The GS-11 full performance level requires completion of a minimum of 1 year of post-MSW degree experience in the field of health care social work (VA or non-VA experience) and licensure or certification in a state at the independent practice level. OR In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required 1 year of professional social work experience in a clinical setting. Demonstrated Knowledge, Skills, and Abilities (KSAs): 1. Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services. 2. Ability to independently assess the psychosocial functioning and needs of patients and their family members and to formulate and implement a treatment plan, identifying the patient's problems, strengths, weaknesses, coping skills and assistance needed, in collaboration with the patient, family and interdisciplinary treatment team. 3.
