  • On-going support, counseling and advocacy for the needs of clients and their families.

  • Provide the majority of services in the client's home or a location of the client's choosing.

  • Acts as primary contact person for not less than eight and no more than 20 clients/families.

  • Development and implementation of action plans for clients per program guidelines.

  • Maintains timely and accurate client files, including care plans, correspondence and case notes.

  • Locate and help secure appropriate housing arrangements for families.

  • Assist clients in identifying un-addressed needs related to benefits, education, housing, medical and other needs.

  • Maintain records and statistics on clients as directed by Program Manager.

  • Assist in program development and quality assurance activities as directed by Program Manager.

  • Create awareness; educate policy makers on issues relevant to the needs of clients.

  • Develop a strong community profile within the homeless services and/or HIV/AIDS community as an advocate for clients.

  • Mandated to report any witnessed or suspected child abuse to the Department of Children and Families.

  • Occasional 24 hour on-call coverage


  • Minimum BA/BS with two years related experience, or master's degree in related field.

  • Bilingual/Bicultural preferred.

  • Ability to provide comprehensive, intensive services to the families with disabilities in a community setting.

  • Valid CT Driver's License, proof of insurance and access to reliable transportation.

Location: New Haven and Hartford, Connecticut The Manager of Case Management Operations is responsible for providing leadership to the New Haven and Hartford Connecticut regions for clinical case management operations. Primary duties may include, but are not limited to: * Develops and implements case management strategies, policies and procedures. * Improves the work flows, processes, and policies in support of CareMore's mission. * Cultivates community connections with an eye towards addressing social drivers of health in the patient population. * Acts as an advisor, giving direction to all case manager staff, resolving issues/problems as needed. * Participates in all department data system enhancements. * Manages and increases the effectiveness and efficiency of Case Management staff through process improvements. * Serves as liaison between case management and other departments. * Hires, trains, coaches, counsels, and evaluates the performance of the team to include RN Case Managers and Community Health Workers. Qualifications Qualifications: * Requires a valid, unrestricted RN license in the state of Connecticut. * Requires a BS/BA degree. * 3-5 years direct experience as a manager; or any combination of education and experience, which would provide an equivalent background is required. * People management experience is required. * Experience in community health/population health/Primary Care is highly preferred. * Experience in case management, working with patients that have complex illnesses preferred. * Experience with transitions of care and coordinating care preferred. * Experience with Medicare and managed care is desired. * Have, or willing to obtain CCM certification within the first year of employment.
