Pay Range: $26.12 - $33.12 (Non-Bargaining Pay Plan, B24/B31 Step 1-13)

Hiring Range: $26.12 - $27.71 (Non-Bargaining Pay Plan, B24/B31 Step 1-4 DOQ)

There is one Case Manager opening with the Yakima County Human Services Department. This position is responsible for supporting community members to access complex system(s), transportation, case management of a variety of social services such as health services, drug and alcohol services or employment and training services.

There will be an exercise using Word/Excel as a part of the interview.

Responsibilities:

Essential Duties:

Conducts comprehensive screenings and diagnostic interviews to obtain information such as: substance use, biological, psychological and social histories; evaluates the information; exchanges information and concerns with appropriate medical and behavioral health organizations and administrative professionals and recommends appropriate services. Engagement is done with both individuals experiencing unsheltered homelessness, and those who are engaged with case management in the Yakima County Jail who have been identified as exiting to homelessness after release.

Maintains a client case record and documents compliance; regularly and reliably updates services in the appropriate project in HMIS; inspects and reviews files for compliance with regulations; completes clinical review forms, and prepares monthly statistical reports.

Provides technical, program, and service advice and assistance to clients, families, caregivers, contractors and service providers; counsels clients on an individual basis; and develops case plans to meet needs.

Promotes and advocates program services. Plans, develops and implements events, presentations, workshops, orientations and community forums. Provides and presents program information to employers, prospective applicants or clients, care providers and the public regarding issues such as: education, homelessness, chemical dependency, treatment alternatives, counseling services, and unemployment issues.

Provide transportation for individuals and families in homelessness to local providers for scheduled appointments and/or to receive urgent services, like shelter, detox, treatment and/or behavioral health.

Facilitate Department of Corrections case conferencing sessions: providing technical, program and service advice and assistance to clients, families, caregivers, contractors, and service providers; counsels clients on an individual basis; and develops case plans to meet needs.

Communicate with individuals and organizations in the community to receive referrals for individuals experiencing unsheltered homelessness in need of case management. Work productively and professionally with referral sources (e.g. local businesses, WSDOT, schools, hospitals) to ensure effective outreach and responsiveness.

Performs other duties as required.

Education & Experience:

Associate Degree or two-year technical certificate in Social Work or related field AND three (3) years of direct service case management or social service program experience; OR an equivalent combination of education, experience, and lived experience which provides knowledge, skills and abilities sufficient to successfully perform the essential duties of the job.

Required:

  • valid WA State Driver's License and proof of insurance, if requested; and,

  • successful completion of a criminal background investigation and general employment verification.

Preferred:

  • client case management experience and/or lived experience;

  • experience using Microsoft Word, Excel, and Outlook;

  • some knowledge of principles, theories, and practices related to human services, homelessness, and behavioral health; and,

  • bilingual in Spanish

Equipment Used: computers, telephones, printer, photocopiers, webcam equipment, projector, other basic office equipment, and operation of a passenger vehicle.

Working Conditions: This position is rated as Sedentary Work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Sedentary work involves sitting most of the time. Jobs are sedentary if walking and standing are required only occasionally, and all other sedentary criteria are met.

Environment: exposure to weather, pathogen exposure, travel, multiple work locations, won in locked down facility(s), field work, may be required to work outside of standard business work hours and at alternative work locations, and may have the potential exposure to hostile/angry individual.

Physical Demands: bending, carrying, handling, lifting, pulling, pushing, reaching, sitting, standing, walking, fingering, kneeling, hearing, smelling, talking, and visual acuity.

Intellectual Demands: ability to multitask, confidentiality, ability to work under pressure, ability to articulate and communicate information in conversations. ability to follow written instruction, and ability to follow verbal instruction.

Knowledge:

  • case management theories, practices and principles;

  • contracting principles and techniques;

  • terminology, laws, rules, and regulations and codes governing area of assignment;

  • technical program delivery methods, systems and techniques such as: court and criminal justice systems unemployment systems, chemical dependency treatment resources, or long-term care service delivery systems;

  • operational methods, principles, issues and service standards of the area of assignment; and,

  • customer service techniques and team building concepts.

Skills (and abilities):

  • using a personal computer and related software applications;

  • operating basic office equipment;

  • determining priorities and making critical decisions;

  • developing initiatives and projects;

  • solving problems, resolving conflict, and building teams;

  • giving presentations and facilitating meetings;

  • establishing and maintaining effective interpersonal relationships with County and other officials, at all organizational levels and with the public; and,

  • communications, oral and written, sufficient to exchange or convey information and to give and receive work direction.
