JOB SUMMARY

The Qualified Developmental Disability Professional shall perform in accordance with ICF/DD Title XIX regulations guidelines. All functions shall be performed in accordance with the established policies and practices of the Federal, State and local policies that governor the Title XIX funding source.

ESSENTIAL DUTIES AND RESPONSIBILITIES

To assess and monitor the health and well being of the individuals served. Consult with medical professionals, observe staff, review medication administration and perform related duties as assigned. Monitor employee performance. Position is responsible for developing and implementing quality assessments, assuring that evaluation and follow-up are completed in a timely manner, coordinating treatment recommendations, monitoring school and day program treatment plans, consumer fund accounts, ensuring that quality outcomes are achieved and advocating for individuals served.

SUPERVISORY DUTIES

Quality Assurance

Group Home

1.Provide quality management support and supervision to assigned house managers.

2.Ensures that residential homes are in compliance with safety and building codes and licensing standards.

3.Provides twenty-four hour on call coverage as needed and as assigned by Program Director.

4.Participate in annual state survey.

5.Participate in CARF accreditation.

6.Participates in the admission and discharge of individuals served.

7.Conducts investigations and prepares reports of findings to Program Director or appropriate Volunteers of America Southeast Louisiana management.

8.Functions as QDDP for all assigned consumers.

9.Attend monthly residential home meetings and Administrative monthly meetings.



  1. Providing training to DSP and Home Managers as needed.

  2. Evacuate with Community Living Services program during hurricane season.

  3. Provide Medicaid and Social Security with all need documentation in a timely manner.


Quality Assurance

1 Manages the development and implementation of treatment and training program

2 Ensures quality outcomes are achieved.

3 Collect and aggregate data quarterly for outcome management reports for all community homes under direct supervision.

4 Collect and aggregate data for the 1st and 25th of the month reports for all community homes

5 Perform quarterly QA home reviews at all of CLS community homes

6 Collect data for driving assessments for all of CLS community homes

Residential Care Functions

1 Ensure residents are treated with respect and dignity

2 Participate in consumer interdisciplinary team meetings, annual IHP's and Quarterly updates

3 Report incidents of neglect or abuse of consumers

4 Report all critical incidents to the Louisiana Department of Health (DHH) within the specified time required by DHH.

5 Perform IDT- interdisciplinary team meeting for residents as needed

6 Ensure residents access the community

7 Ensure resident have access to medical services

8 Assist as support staff if needed to provide Direct Support Care.

9 Assist residents in developing a circle of support

Charting and Documentation

1 Complete monthly and quarterly reports and submit to the Program Director

2 Charting in resident files per event and quarterly

3 Perform routine charting duties as required. (i.e. discharge from hospital, admit to hospital, incidents with residents, change in behavior or diagnosis.

KNOWLEDGE, SKILLS AND ABILITIES

This position requires the incumbent to have a B.A. degree in human services discipline and two years of experience working with adults who have mental illness or developmental disabilities. Must have demonstrated supervisory experience and the ability to effectively manage people and understand Title XIX regulations regarding community based residential homes. Requires excellent verbal and written communication skills and proficiency with using a personal computer is desirable.

PHYSICAL REQUIREMENTS

Physical requirements include sitting, standing, bending, pushing, pulling, squatting and lifting.

  • The ability to safely operate a motor vehicle to transport oneself, consumers, and program supplies as necessary.

  • Move and lift light objects up to 30 pounds such as mail, supplies, files, and equipment.

  • Operating office equipment requiring continuous or repetitive hand/arm movements.
