Baccalaureate degree in social work, psychology, or closely related field, computer proficiency, valid Colorado drivers license, good driving record, and access to transportation during working hours are required. Ability to work in Walsenburg with occasional assignment in/travel to Trinidad.
Preferred: Experience working with a multi-disciplinary team. Experience working with adults who have severe and persistent mental illness. Experience providing case management services to adults, and community systems navigation desired. Ability to speak Spanish desired but not required. Certified Addiction Counselor (CAC) training and experience working with co-occurring disorders desired. Knowledge of/experience with HUD housing rules and regulations preferred.

The case manager maintains a caseload and is available to conduct case management services for clinicians, when needed. The case manager is also responsible for enrollment of all new clients. The case manager assist with outreach to individuals when needed. Services may be provided in home, in a correctional setting, in conjunction with services at Health Solutions or at other locations in the community that provide a setting of natural support for the client. Duties can include case management services, behavioral health counseling and outreach, and documentation.
Makes appropriate referrals to address mental health and substance use needs, and provides linkages to community resources
In cooperation with the Health Solutions Housing Case Manager, administers, monitors, and manages all aspects of housing vouchers for Huerfano and Las Animas counties, including travel to client homes
Conducts client home and community visits
Ensures clients attend their scheduled appointments in the community and at Health Solutions;
Identifies stressors and precursors that led up to the current situation, and/or crises
Identifies coping mechanisms and natural supports that may be available
Enhances problem-solving skills, independent living skills, and socialization skills
Maintains competency and proficiency with the agencys electronic health record
Maintains accurate records
Provides support for clients to remain in home and community to avoid more restrictive living arrangement
Obtains information and coordinates with community providers when care is split
Performs other job-related duties assigned by the director or his/her designee
Wage: $44,969 annually (hourly, non-exempt, 40 hour work week)
