Program and Agency Description

Volunteers of America Community Family Supports program is looking for dedicated and energetic individuals to join our team as a Mental Health Case Manager. Mental Health Targeted Case Management provides services supporting children and adults with mental health concerns. The goal of Case Management is to keep clients in their home and community. Case Managers help coordinate services to support areas such as: mental health, social, education, health, vocation, recreation, volunteer, advocacy, transportation, and legal.

Volunteers of America Minnesota is a nonprofit health and human services organization that annually serves more than 25,000 children, adolescents and their families, older adults, students, persons with disabilities and special needs, and adults completing incarceration. Since 1896, we have helped people gain self-reliance, dignity and hope.

Essential duties and responsibilities

  • Provide services in the four core case management components-Assessment, Service plan Development, Referral and Linkage and Monitoring and Coordination of Services

  • Coordinate with a mental health professional to assure the completion of a mental health assessment of the child's and family's needs and to develop individual treatment plans.

  • Develop and update the child's individualized plan (IFCSP). Coordinate, monitor, and evaluate services that are outlined in the plan for appropriateness, progress, and continued service need.

  • Coordinate services with parent, child, and other members of the multi-disciplinary team.

  • Develop and utilize available area resources and make appropriate referrals.

  • Meet with the family and/or program participant as described in the treatment plan and program policies.

  • Encourage and support parents in the parenting of their children.

  • Keep necessary case notes documenting contact with program participant, family and collateral contacts.

  • Maintain program participant case files.

Requirements

Education/Experience:

  • Bachelor's degree in one of the behavioral sciences or related fields from an accredited college or university

  • 2000 hours of supervised experience in the delivery of clinical services in the treatment of mental illness.

  • Computer proficient

  • Ability to travel within Stearns, Benton and Wright counties.

  • Proven skills: Prioritizing, Multi-tasking, Organized, Interpersonal skills, Networking.

Preferred Candidates will be licensed at the Mental Health Practitioner level.

Follows the SCH Nursing Professional Practice Model Compass to achieve the hospital mission if improving the health and quality of life in the community that reflects the healing mission of Jesus. In addition, helps drive change by identifying areas of performance improvement and incorporating evident-based practice. Provides age appropriate care to patients and families. Promotes individual profession growth and development by meeting requirements for mandatory/continuing education, skills competency, supports department-based goals which contribute to the success of the organization, serves as preceptor, mentor and resource to less experienced staff. Participates in peer review process. EDUCATION: Bachelor#s or higher degree in nursing required. EXPERIENCE: Require two or more years of clinical experience as a registered nurse. Experience working with an electronic health record. LICENSES AND CERTIFICATIONS: Current licensure as a registered nurse in the State of Minnesota required. Current American Heart Association (AHA) BLS Healthcare Provider card required per established policy. Current ACMA Case Manager Certification - RN or obtained within five years of hire.
