Essential Duties and Responsibilities:
Determines the patient's need for Plan of Care (POC) assessing needs.
Assist the LCSW and patient in identifying and meeting establish goals.
Based on identified needs, assist the patient with acquiring necessary community resources.
Maintain an EHR record of the patient's progress by documenting patient/Case Manager interaction and profess or lack of.
Develop a therapeutic relationship with patient and caregivers to foster positive patient outcomes.
Coordinates & executes activities resulting in assisting Viemed patients with compliance with respiratory equipment.
Works with Clinical Team of differing states to assist patient with identified needs.
Refers to appropriate reporting agencies for protection of patients.
Attends meetings to provide education on options available for patients and provides updates.
Attends mandatory staff meetings.
Attends clinical trainings to enhance knowledge base for providing services.
Assists patients with navigating medical options within the medical system.
Monitors 02 escalations, prevoid, and pick up reports to identify needs and provide support.
Assists patient with scheduling transport and setting up medical appointments.
Maintain operations by following policies and procedures, participating in quality reviews and reporting needed changes.
Provides social services needed for transitioning patients from in patient care to a home setting.
Some travel required.
Other responsibilities and projects as assigned.
Qualifications:
Associates or Bachelors Degree
Provide license number if licensed.
Confirmed State Licensed or certification to practice or equivalent.
Experience working with medical insurance plans.
Preferred- Medical experience with Geriatric Patients or those with Chronic Illness.
Valid Driver's License.
Preferred- Telehealth & Telephonic Experience.
Good listening and people skills.
