Program Overview:No single entity can respond to the complex needs of youth with serious emotional and behavioral challenges as well as youth who are experiencing the symptoms of substance use disorders. TREE House creates an environment where youth and families can come to get needs met in one spot. Our mission is to enhance and expand comprehensive treatment, early intervention and recovery support services to adolescents (ages 13-17), transitional aged youth (ages 18-24) and their families/primary caregivers with Substance Use Disorders and/or Co-Occurring Disorders. Our services include: Intensive Outpatient Treatment for those suffering with a substance use disorder; Level I Outpatient treatment for Recovery or Early Intervention needs; individual and family counseling; Wrap services with a full treatment team to provide support for the entire family; in-home services and community programs that strengthen families. With the growing epidemic of Opioid Use Disorders and other illicit drugs, our program services to address the pervasive problem of substance use among youth in the North Metro Atlanta area and the urgent need to fill gaps in treatment and services specific to this population.Position DescriptionCase Manager will provide guidance, education, and support to youth & young adults, as assigned to ensure goals related to education, vocation, living situation, well being as well as other life skills are developed and met. This positions primary duties include but are not limited to the following: * Displaying positive role-modeling to members * Promote self-esteem * Promote healthy boundaries * Promote good hygiene and appropriate personal appearance among young adults * Reinforce socially acceptable behaviors (ex: manners, eating habits, personal safety)* Teach and reinforce thoughtful decision making * Teach and reinforce professionalism in work tasks and employment * Assessing members' history of trauma, strengths and needs;* Administer CANS (Child & Adolescent Needs & Strengths)* Aware of ACE score and implications on service planning * Develop & monitor Individualized Service Plans * Coordinating Services and Collaboration * Meet with assigned member at minimum monthly * Monitor, schedule, facilitate and/or participate in Team Meetings for members. *
  • Collaborate and communicate with other programs in the CHRIS 180 continuum in the provision of services to members * Collaborate and consult with other providers and entities in the provision of best care services to members * Monitor the provision of services provided * Management of Caseload * Make necessary partnerships to meet member's health, well-being, education, mental health, and employment and future planning needs (i.e. Department of Public Health, Department of Family & Children Services, Georgia Department of Labor, employment staffing agencies, local colleges and tech schools, etc.)* Research and/or attend trainings to educate self on best practices to provide adequate services to members * Employ tools to maintain compliance with governing agencies or entities (i.e. DBHDD)* Manage budget and ensure appropriate spending * Oversee members progress and program participation * Documentation Requirements * Provide member updates in Case Notes * Complete Incident Reports as needed prior to shift end * Monitor completion of deadlines (i.e Monthly Summary Reports)* Provide reports to governing/referral agencies regarding participation of member * Performance and Conduct * Maintain confidentiality of member information * Maintain appropriate professional boundaries * Follow the CHRIS 180 Code of Ethics * Follow mandated reporting requirements of GA Code * Support and maintain an environment of emotional and physical safety for members, staff and other stakeholders at all times * Demonstrate customer service;* Promote positive image of CHRIS 180 programs at all times * Work as a contributing and collaborative team member * Respond to consumer and peer needs in a timely fashion * Answer telephone calls and emails accordingly * Respond to requests for information from stakeholders in a timely fashion * Maintain Personal Information and Training;* Attend and participate in staff meetings as scheduled * Attend and participate in required trainings as scheduled * Maintain required certifications * Review, understand and responsible to program policies * Provide documents in a timely fashion to Human Resources * Report changes in status to HR as required by policyQualifications/Competencies * Experience working with youth and transition age young adults with mental health and/or substance abuse diagnosis (co-occurring)* Highly organized and self-sufficient * Bachelors Degree* in the field of social services (Ex. Psychology, Social Work, Sociology)Schedule * The Case Manager works an average of 80 hours bi-weekly with a rotating on call schedule. * The work schedule requires (2) Saturdays per month and flexibility to meet the needs of the program and availability on an emergency basis. * Due to schedule or other operations necessities, the organization may transfer an employee to another location or shift, either on a temporary or permanent basis as this position is an employee of CHRIS 180, not the program.Reports to: TREE House Project Manager
