As part of the Stonerise Healthcare team, the Case Manager is to be responsible for providing leadership and management of clinical case management, clinical social work, utilization management, and discharge planning. The Case Manager assumes complete oversight of the precertification to skilled nursing centers as well as continued coverage throughout discharge. The Case Manager acts as an intermediary between the insurance provider, the health care provider and the patient.

Area of assignment will be Morgantown/Monongalia County and surrounding areas.

Job Functions and Responsibilities:

  • Communicates with Interdisciplinary team daily to process managed care referrals, authorization for services, and maximize patients benefit for continued coverage throughout discharge.

  • Completes insurance verification of benefit and initiate authorization for admission.

  • Completes managed care assessment in PCC to summarize patient coverage and plan.

  • Participates in weekly IDT meetings to capture patient progress towards goal of next level of care.

  • Provides concurrent updates to managed care organizations timely to maximize benefit and length of stay.

  • Knowledgeable of managed care contracts to incorporate services provided for appropriate reimbursement.

  • Provides clear communication to IDT regarding patients benefit and last covered day of services.

  • Completes managed care assessment to finalize services for billing accuracy.

  • Responsible for organizing and maintaining documentation of clinical record for submission to managed care organizations.

  • Establishes and maintains a positive and professional relationship with current and potential providers and works to ensure patient and provider satisfaction.

  • Maintains comprehensive working knowledge of Stonerise Healthcare markets and promotes a working relationship among its affiliates.

  • Knowledge of Corporate Business Management

  • Ability to communicate effectively verbally and in writing.

  • Experience in Managed Care authorization and/or review.

VIEW JOBS 1/13/2020 12:00:00 AM 2020-04-12T00:00 What You Will Do: As a hospice case manager for Amedisys, you will have a special opportunity to provide, coordinate and direct the provision of hospice nursing and palliative care to terminally ill patients in their home. You will closely collaborate with the attending physician, patients and their families, and other members of the patient care team to provide and maintain continuity of patient care to achieve excellent pain and symptom management and high quality end-of-life care for the patient. * Assess and reassess all physical, environmental and emotional factors to determine hospice needs for patients in the home setting * Works in cooperation with the family/caregiver and IDT to meet the emotional needs of the patient/family/caregiver. * Utilize a combination of agency resources and nationally recognized standards of practice to achieve excellent pain and symptom management and high quality end-of-life care. * Coordinate the total Plan of Care and maintains continuity of patient care by liaising with appropriate staff and the physician. * Actively participates in Interdisciplinary Team (IDT) conferences. * Provide supervision and evaluation of aides and nurses. * Attend funerals/calling hours when appropriate as assigned by the bereavement coordinator. Required: * Current and unencumbered license to practice as a Registered Nurse specific to that state the employee is assigned to work by the Company. * Minimum of one (1) year of experience as a registered nurse * Current CPR certification * Valid Driver's License and reliable transportation and agency required liability insurance to complete home visits Preferred: * Bachelor of Science in Nursing (BSN) degree * Previous home health or hospice experience * Certified Hospice and Palliative Nurse (CHPN) Amedisys is an equal opportunity employer.
