Starting Compensation:

24.36 USD Per Hour

Working Location:

San Jose: North San Jose Center (In-person position)

Working Hours/Shift:

Monday-Friday, AM Shift

What you bring to SBHG (qualifications):

• HS degree and six (6) years of direct service experience in the Mental Health field

• Associates degree and four (4) years of direct service experience in the Mental Health field OR

• Bachelor's degree and two (2) years of experience working in the mental health field

• Valid California Driver's License

• Meets Agency driver eligibility criteria.

How you will make a difference (job overview):

This position has primary responsibility for providing oversight and care management to the clients referred to IHOT programs. The Case Manager will provide services that may include case management/linkage, assessments, screenings, treatment planning/monitoring, and rehabilitation services to the clients and their families.

Division/Program Overview:

• Increase access to care through outreach and engagement

• Provides coaching, role modeling and psychoeducation to individuals and families.

• Working as a multidisciplinary paraprofessional team

• Provides services to individuals throughout Santa Clara County
