24.36 USD Per Hour

Working Location:

San Jose: North San Jose Center (In-person position)

Working Hours/Shift:

Monday-Friday, AM shift

What you bring to SBHG (qualifications):

  • HS diploma and six (6) years of direct service experience in the Mental Health field OR

  • Associates degree and four (4) years of direct service experience in the Mental Health field OR

  • Bachelor's degree and two (2) years of experience working in the mental health field

  • Valid California Driver's License.

How you will make a difference (job overview):

This position has primary responsibility for providing oversight and care management to the clients referred to Assertive Community Treatment programs. The Case Manager will provide services that may include case management/linkage, assessments, screenings, treatment planning/monitoring, and rehabilitation services to the clients and their families.

Division/Program Overview:

  • The program sees adults (18+) and older adults.

  • It is a FULLY community-based program - all services happen at a person's home or community.

  • The clients have complex needs - medication, housing, vocational, independent living and mental health.

  • The intensity for services for each client is about 10 hours per month and once we are fully up and running, we'll have a census of about 50 adults.

  • We are preparing to also start an Adult Outpatient Program which this position would oversee as well. Eventually, this position will have all our Adult programs, including IHOT.

Grade State Range: 24.36 - 36.54 USD Hourly
