The Case Manager will be responsible for assisting participants in the STAR program to enhance their home, health, community, and purpose through developing a person-centered Integrated Family Case Plan inclusive of strategies for recovery. The Case Manager will provide strength-based case management services including helping individuals with acquiring needed resources and navigating multiple social service and health systems. Additionally, the STAR Case Manager will provide intensive system coordination for participants involved in STAR to address whole health needs inclusive of MAT, chronic and acute health problems and problems stemming from the criminal justice system.

Essential Functions

  • Provide a comprehensive case management assessment that addresses life domains such as housing, finances, transportation, legal services, vocational, employment, health care, and family strengths/needs.

  • Develop an Integrated Family Case Plan that is person-centered and includes strategies for recovery. The plan shall identify priorities, desired outcomes and the strategies and resources to be used in obtaining outcomes based on the case management assessment.

  • Link to appropriate care and resources in the community including resources that address specialized needs, such as agencies providing services related to HIV/AIDS, mental health disorders, chronic and acute health problems and problems stemming from involvement with the criminal justice system.

  • Promote the STAR individual's recovery to his/her fullest potential by facilitating assessment of skills, including literacy, educational opportunities (GED, certification programs), job readiness skills, vocational training, employment placement and retention and career development.

  • Collaborate/communicate with systems partners including prisons/jails, DMHAS' Interim Managing Entity (IME) to ensure coordination of care.

  • Documents all clinical activity and correspondence in accordance with Center policy, state regulations, and Joint Commission standards.

  • Participates in supervision and team meetings as well as collaborate with other treatment providers to monitor and discuss clients' progress and needs.

  • Tracks essential data and performance improvement measures and provide regular reports to the Program Director - CARE.

  • Contributes to agency performance improvement efforts to assure treatment is in accordance with state regulations and accrediting body standards.

  • Performs other related duties as assigned.

Essential Qualifications

  • Bachelor's level CADC, LCADC, or Master's in health, psychology, counseling, social work, education or other behavioral health profession.

  • Possess knowledge, skills and experience necessary to competently perform case management activities.

  • Minimum of three (3) years' experience in providing services to individuals and their families/caregivers who have mental health disorders and/or co-occurring substance use disorders.

  • Has experience in working across cultures.

  • Must possess knowledge in formal/informal community systems and resources.

  • Must have knowledge of Wellness and Recovery Principles, Person Centered Care, Trauma Informed Care and Integrated Healthcare.

  • Must possess a valid NJ driver's license in good standing.

  • Skill in the operation of office equipment, including but not limited to computers, printers, copiers, calculators, and other job specific equipment.

  • Ability to work collaboratively and build relationships with colleagues, coworkers, vendors, contractors, and the public.

  • Ability to accurately prepare and maintain files, records, reports, and logs.

  • Skill in using computer applications including electronic health record, spreadsheets, databases, and word processing software.

  • Ability to identify and maintain confidential information.

  • Ability to comprehend, express and exchange information in English over the phone, in person, in writing or through other communication or electronic devices.

Preferred Qualifications

  • LCADC/CADC

  • Knowledge of community resources and services and the ability to collaborate with other professionals.

  • Bilingual (English/Spanish)

  • Lived experience in mental health and/or substance use disorders and in recovery for a minimum of 5 years is desired
