QUALIFICATIONS

• LVN or RN with Knowledge of healthcare reimbursement, case management and discharge planning is required.

• Understanding SNF regulatory requirement

Team Touchstone Directors of Care Solutions/Case Managers are responsible for the following:

• Exercise clinical expertise, knowledge and decision making to ensure successful discharge planning and care management

• Ensures necessary information is communicated to IDT, direct care staff, patient and responsible party/caregiver

• Facilitates the 72-hour and Care Connect meetings

• Responsible for the ongoing communication to the case management team

• Coordinates Care reviews

• Guarantees patients are monitored for change of condition

• Collaborate with the interdisciplinary team to develop individualized plans of care

• Leads successful transition of care though continued recovery once at home

• Provides education and support to patients and families about care processes, discharge expectations and resources

• Serve as member of the QAPI committee

• Director of Care Solutions is aware of the Balanced Scorecard and how their position contributes to success in Balanced Scorecard measures
