The Opportunity:

Overview:

This is a Limited Duration for two (2) positions. This pilot limited-duration position is scheduled through June 30, 2025 with the possibility of being extended, but can end at any time.

THIS WORK MATTERS!

The High Risk Case Manager 3 - Limited Duration position exists to provide case management services for high-risk youth and young adults with developmental disabilities-the state Office of Seniors and People with Disabilities contracts with Multnomah County to provide case management services. Case management aims to access, coordinate, and assure the delivery of services and supports required by individuals with developmental disabilities. Case Management will assist eligible individuals under their individualized plan in accessing needed medical, social, educational, and other services. Services include (1) determination of appropriateness and need for developmental disabilities services; (2) evaluation of individual needs; (3) re-evaluation of individual needs; (4) development of an individual service plan; (5) assisting the individual in accessing needed services; (7) assisting the individual in accessing crisis services; (8) coordination of protective services; investigation and documentation of complaints; (9) coordination of services with other agencies who are involved with the individual (i.e., adult and Family Services, Children's Services Division, Vocational Rehabilitation Division, Senior Services Division, Social Security Administration, Veterans Administration, Department of Education, et cetera).

As the High Risk Case Manager 3 - Limited Duration, other duties will include:

  • Exercise significant independent judgment and initiative. Apply understanding of complex rules to secure placement and support for youth who are involved with numerous systems

  • Work with a high degree of autonomy and independence in assessing and authorizing program services to individuals eligible for IDD supports

  • Initiate and Perform complex case management duties for clients impacted by intellectual and/or developmental disabilities, including assessing, evaluating , and documenting eligibility for full spectrum Federal and State program services and benefits

  • Coordinate placements and services and Monitor services/support via home visits, service provider visits and phone calls to ensure that they are receiving appropriate financial, medical, training, educational, transportation, and other identified/agreed to service/supports

  • Take lead in transitioning individuals to other public and private resources as their needs change as they age from children to adult services

  • Coordinate extensive support teams from multiple services sectors (Child Welfare, Mental Health, Foster providers. etc.) to develop and complete Individual Service Plans

  • Report possible protective services issues and provide initial information to assist the investigation

  • Maintain case records and document actions taken in accordance with federal, state, local policy; develop, submit, and modify necessary documentation to implement/change/adjust payments and program benefits

  • Facilitate annual Service Coordinator ONA Review assessment

  • Independently identify statewide resources appropriate for individuals

Minimum Qualifications/Transferable Skills*:

Education/Training:

  • An Associate degree from an accredited college or university or equivalent practical experience

  • Three (3) to Six (6) extensive experience working with individuals from the Intellectual and Developmental DIsability Community (IDD)

  • Ability to promote a culture of respect, inclusiveness, and appreciation of diverse perspectives, backgrounds, and values

  • Must have a valid driver's license by the time of hire

  • Must pass a criminal background check
