The Case Manager Senior position serves under the diversion courts, which serves Community Court, Forensic Diversion, and Mental Health Court where participants are offered intensive case monitoring, care coordination, and advocacy, which supports individuals who are experiencing severe mental health barriers that cause them to be intertwined with the criminal justice system in which the individual may be looking at significant jail or prison time if they are not diverted to community based treatment as opposed to custody time. The Case Manager Senior will assess, support, implement, manage, advocate, monitor, report, and sanction individuals participating in Mental Health Court by using evidence based tools, like motivational interviewing, to help ensure that participants whose barriers may be severe and persistent mental illness, poly-substance abuse, developmental and/or functional cognitive needs, chronic homelessness, immigration concerns, high criminality, high acuity, or low incomes are engaging in and connected to treatment that supports them through dual diagnosis treatment, with a trauma informed, cultural sensitive, or LGBTQ lens.

As the Case Manager Senior, other duties will include:

Providing Liaison, Complex Case Management and Coordination of Services

  • Interview, and assess defendants to determine level of mental health, substance use, criminal risk factors, social services and sustainability for community or residential treatment.

  • Provide long term specialized, complex case management, and coordination across the forensic, civil and social service systems.

  • Provide and directly facilitate referrals for mental health, chemical dependency, developmental disability and social services needs.

  • Maintain community and telephone contact with the client's mental health or addictions treatment providers regarding compliance, progress, and need for interventions.

  • Provide interim crisis support and intervention to participants experiencing psychosis.

  • Meet with participants in the office or community to engage, counsel, intervene and promote compliance with court orders and treatment recommendations.

  • Facilitate coordination between opposite systems of criminal justice and social services by possessing a complete and thorough understanding and knowledge of the criminal justice, mental health, addictions, language, systems, and culture.

  • Make weighted clinical decisions and recommendations for participants terminations, sanctions, treatment, housing, payee necessity, curfew, graduations, court dates, and phase levels in Mental Health Court.

  • Provide reports to the court and probation department regarding compliance with social services, treatment recommendations, and crisis interventions.

  • Support participants with hostile, aggressive, substance induced, developmental challenges, traumatic brain injuries, acute psychosis, high criminality, co-morbid populations, and individuals dealing with chronic homelessness.

  • Provide engagement, transition, and community support for participants.

Transition and Navigation

  • Provide support to clients as they transition from corrections to community services.

  • Review criminal history, assess risk of charges and/or risk of re-offending and help coordinate appropriate interventions and community support transitions.

  • Serve as a clinical resource for the Mental Health Court team, community partners and families of participants.

  • Serve as a legal and community resource for medical and psychiatric systems.

  • Serve as an internal consultant to other Behavioral Health Programs internally and externally, providing professional counsel and expertise.

Skill Development, Documentation, Navigation, Organization and Professionalism

  • Develop and implement court reports that convert into court orders making recommendations and sanctions for the participants.

  • Develop Evolv crisis alerts when individuals become a risk to themselves or others.

  • Document Evolve notes, with updates, changes in address, phone numbers and natural supports.

  • Complete a weekly tracking sheet documenting contacts and interventions with participants.

  • Observe client confidentiality per person, agency and Health Insurance Portability and Accountability Act rules.

  • Continually improve professional skills, knowledge, and ability in order to achieve expert status by attending internal and external trainings along with networking to learn about new community resources.

TO QUALIFY:

Minimum Qualifications (REQUIRED):

Education/Training: Equivalent to an Associate's degree from an accredited college or university with major course work in social science, behavioral science, or related field and three (3) to six (6) years of experience that demonstrates the ability to perform the duties of the position. (Equivalency 5-8 years of qualifying training and/or experience); AND

Experience: One (1) year of case management experience; AND

Certification: Must meet the standards of Qualified Mental Health Associate - (QMHA) with the Mental Health & Addiction Certification Board of Oregon (MHACBO) - more information can be found at MHACBO website. Current QMHA I, QMHA-R, or obtain within 90 days of hire.

  • Valid Driver's license.

  • This position requires a background investigation, which may include being fingerprinted.

Preferred Qualifications/Transferable Skills*: You do not need to have the following preferred qualifications/ transferable skills to qualify. However, keep in mind we may consider some or all of the following when identifying the most qualified candidates.

  • 1-2 years experience providing case management services

  • Knowledge of issues faced by those who experience behavioral health challenges and a basic understanding of the full continuum of care for those in need of services.

  • Lead with race through actions and advocacy with internal programs, with community partners and across departments throughout the county.

  • Demonstrate the ability to flex style when faced with myriad dimensions of culture in order to be effective across cultural context.

Teleworking: At this time, this position is designated to "routine telework," meaning you will be working in the community or at home most of the time; a hybrid schedule also is available. As with all routine telework positions, employees may be required to come into the office on occasion. The designation of Routine Telework may be subject to change at a future time. Currently, all employees must reside in Oregon or Washington; the county cannot support ongoing telework from other locations.
