Case Manager

You will be a vital role in managing and directing a patient focused clinical plan of care developed and implemented by a team of professionals that improves quality of life and leads to a safe and timely discharge.

Responsibilities

  • Develop and implement a patient specific, safe and timely discharge plan

  • Perform verification of utilization criteria reviews

  • Build relationships and coordinate with payor sources to assure proper reimbursement for hospital provided services, promote costs attentive care via focus on resource management within the plan of care

  • Demonstrates compliance with facility-wide Utilization Management policies and procedures. Coordinates UR compliance with Quality Management to assure all licensure and accrediting requirements are fulfilled

  • Maintains fiscal responsibilities. Assures department is identifying and negotiating the fullest possible reimbursement to maximize insurance benefit coverage for the patient. Reviews insurance verification form to minimize risk.

  • Facilitate multi-disciplinary team meetings including physicians, nurses, respiratory therapists and rehabilitation therapists

Qualifications

  • Registered Nurse (RN) or Licensed SW

  • Adequate experience (2-3 years) in an acute hospital / medical case management setting and confidence to manage and direct a plan of care for chronically critically ill populations

  • Experience in care management, discharge planning, and utilization review

  • Working knowledge of the insurance industry and government reimbursement

  • Passion for providing superior quality of care and excellent organizational and communication skills

VIEW JOBS 5/11/2020 12:00:00 AM 2020-08-09T00:00 <p>Case Manager is responsible for providing psycho-social services to patients and their families, including completing social assessments of the patient and family, with a focus on the discharge planning and continuum of care needs. </p><p><strong>ESSENTIAL FUNCTIONS:</strong></p><ul> <li>Consultation, discharge planning, education, counseling, advocacy, info. referrals to meet the emotional, social and financial aspects of care</li> <li>Provides crisis intervention to patients/families with psycho-social needs, and coordinates and facilitates development of a discharge plan of care</li> <li>Provides patient/family services via case finding referrals from physicians and healthcare team members</li> <li>Serves as an expert, acts as a liaison between patients and community resources</li> <li>Identifies patient and family psycho-social and environmental needs related to admission, diagnosis, treatment and discharge</li> <li>Works collaboratively with other members of the interdisciplinary team in developing and implementing a comprehensive discharge plan</li> <li>Makes appropriate referrals for community services; may provide inpatient emotional support and counseling, crisis intervention, and bereavement services</li> <li>Assesses and intervenes in situations involving child or elder abuse/neglect, domestic violence</li> </ul><p><strong>Requirements</strong></p><ul> <li>Bachelor's Degree in Social Work required</li> <li>Master's in Social Work preferred</li> <li>One year of social work experience in hospital or related health care setting, Emergency Department experience preferred.
