The Case Manager coordinates a continuum of care for defined population of patients from pre-admission through post discharge. He/she assures that patients and families proceed efficiently through the course of hospitalization.

The Case Manager is responsible for assessing the appropriateness of inpatient admissions and/or observation status and the medical necessity of continued stay, ensuring appropriate resource utilization, and communicating with outside reviewers. In addition, the Case Manager coordinates accurate, timely and effective discharge planning based upon assessed needs and provides social services including resource identification and referral and promotes safe and continuous post hospital care. He/she also assists patients and their families in coping with psycho-social problems related to acute and chronic illness, treatment regime and adjusting to an altered lifestyle.

This position will collaborate with physicians and other patient care departments. The population served will be pediatric, adolescent, adult, and geriatric.


A. Registered Nurse with current California license or Bachelor of Social Worker or Master of Social Worker required.

B. BCLS required (within 30 days of employment)

C. Minimum 3 years experience in an acute care hospital setting preferred.

D. Previous experience in case management, care management, or utilization management/discharge planning/social service in an acute hospital setting preferred.

E. Knowledge of clinical and social factors that affect the patients functional satus at discharge, knowledgeable of community resources to meet post discharge clinical and social needs and assessment skills preferred

F. Excellent communication skills, both written and verbal required.

G. Must possess strong interpersonal skills and proven ability for same with adeptness at handling sensitive issues and conflict resolution required.

H. Weekend availability required

The incumbent will review assigned medical records to determine if documentation meets HEDIS measurement specifications set by NCQA and enter all findings into the NCQA certified software on a daily basis. In addition, the incumbent must meet daily productivity and data entry accuracy goals, participate in medical record collection, request and abstract additional medical records found through the pursuit process. Position Responsibilities: * Conducts medical record review according to the current NCQA HEDIS® Technical Specifications. * Enters all findings into the NCQA certified software. * Requests medical records as appropriate from practitioner offices, clinics, hospitals, other clinical facilities. * Identifies and effectively communicate trends found in medical record review to the HEDIS team. * Attends weekly and ad-hoc meetings and trainings as needed. * Abides by all HIPAA and associated patient confidentiality requirements. * Other projects and duties as assigned. Required Skills * Read and interpret medical records for chart audits and abstraction. * Accurately enter data; all nurse abstractors/overreaders are required to pass inter-rater reliability testing for all hybrid measures at a 5% error threshold. * Navigate databases with instruction. * Telework; provide own internet connection and safe working environment. * Excellent written and verbal communication skills. * Work with a diverse team. * Excellent organizational skills. * Utilize and access computer and appropriate software (e.g. Microsoft Office; Word, Excel, Outlook, PowerPoint) and job specific applications/systems. Required Experience Experience & Education: * Current, unrestricted LVN license is required, RN preferred to practice in the State of California. * 3 years of nursing experience required. * 3 years knowledge of HEDIS® preferred or 3 years of medical records management and/or QI experience with proven ability to read and interpret medical record data for chart audits or abstractions required. * Needs to have means of transportation as may be required to work away from the primary office 1% of the time or more to provider offices, clinics, or hospitals.
