The Case Manager coordinates a continuum of care for defined population of patients from pre-admission through post discharge. He/she assures that patients and families proceed efficiently through the course of hospitalization.
The Case Manager is responsible for assessing the appropriateness of inpatient admissions and/or observation status and the medical necessity of continued stay, ensuring appropriate resource utilization, and communicating with outside reviewers. In addition, the Case Manager coordinates accurate, timely and effective discharge planning based upon assessed needs and provides social services including resource identification and referral and promotes safe and continuous post hospital care. He/she also assists patients and their families in coping with psycho-social problems related to acute and chronic illness, treatment regime and adjusting to an altered lifestyle.
This position will collaborate with physicians and other patient care departments. The population served will be pediatric, adolescent, adult, and geriatric.
A. Registered Nurse with current California license or Bachelor of Social Worker or Master of Social Worker required.
B. BCLS required (within 30 days of employment)
C. Minimum 3 years experience in an acute care hospital setting preferred.
D. Previous experience in case management, care management, or utilization management/discharge planning/social service in an acute hospital setting preferred.
E. Knowledge of clinical and social factors that affect the patients functional satus at discharge, knowledgeable of community resources to meet post discharge clinical and social needs and assessment skills preferred
F. Excellent communication skills, both written and verbal required.
G. Must possess strong interpersonal skills and proven ability for same with adeptness at handling sensitive issues and conflict resolution required.
